194 Visit of Consultation TeamsE. The final written report1. The requesting diocesan bishop should receive a writtenreport within one month following the conclusion ofthe consultation.2. Each Team member will prepare a report on those areas ofthe program assigned to him or her. Duplications or possiblecontradictions will be minimized through consultationamong Team members. Each Team member should send atyped copy of his/her report of assigned areas of responsibilitydirectly to the Team chair within one week of the conclusionof the consultation.3. The Team chair will assemble an integrated report, objectiveand based on fact, and a brief introduction tothe report.4. Each section of the report should consist of three parts:- A general description- Commendations (highlighting program strengths)- Recommendations (action to bring formation and postordinationdiaconal life and ministry into conformity withthe provisions of the 2004 National Directory)5. The Team Report is mailed to the Secretariat where it isreproduced and distributed to the Committee members fortheir comments, and approval/disapproval When approved,the report will be sent to the Diocesan Bishop under a coverletter from the chairman, BCD. The report then becomesthe property of the requesting diocesan bishop, to be utilizedas he sees fit.

The requesting diocese is expected to cover the following costsof Team members:- Transportation- Accommodations- Miscellaneous expenses (meals, airport transfers, etc.)- Stipend of $50 per day for each team memberB. Consultation Team members will submit supporting documentsto a person designated by the requesting bishop within seven daysfollowing the Team’s visit Reimbursement of Team members andpayment of the stipend should be accomplished within thirty daysfollowing completion of the consultation visit. A suggested reimbursementform is appended to this Policy Statement as Appendix C.C. As a matter of justice, the wife of the deacon is considered anindividual member of the Consultation Team. Her stipend will beindependent of that of her deacon husband.
