The text of the December 2004 Sixth Revision of the Bishops’Committee on the Diaconate (BCD) Policy Statement: Visit ofConsultation Teams to Diocesan Diaconate Programs.2. A Self-Study InstrumentThe Sixth Revision of this BCD Policy Statement is based on the2004 National Directory for the Formation, Ministry, and Life ofPermanent Deacons in the United States.The Policy Statement is provided as a guide for a diocesan bishop whowishes formally to request a visit of a BCD Consultation Team. Sucha visit is an opportunity for dialogue, discussion, and consultationwhich is summarized in a Consultation Team report to the requestingbishop. The report is confidential in nature. It becomes theproperty of the requesting bishop to be distributed and used at hisdiscretion. It is offered as a service to a bishop and those to whomhe has given the responsibility for the formation of deacons and thepost-ordination life and ministry of deacons.The Self-Study Instrument is a guide for a diocese to reflect on theexperience of the diaconate and a self-critique of the degree ofimplementation of the provisions of the United States Conferenceof Catholic Bishops’ 2004 National Directory for the Formation,Ministry, and Life of Permanent Deacons in the United States. Such aself-study could be implemented at times when the need becomesevident or as part of the preparation for a visit by a BCDConsultation Team. In that case, the self-study should be accomplishedin advance of requesting such a visit. The results should bepart of the materials provided to the Consultation Team as they preparefor their visit. The provisions of Appendix I (Specific NormativeElements in Diaconal Formation) to the National Directory apply.The members of the Bishops’ Committee on the Diaconate hopethat these documents will help to strengthen diocesan programs offormation and post-ordination continuing education and spiritualformation of deacons.

The diaconate is an integral part of the entire ChristianCommunity. By means of the document National Directory for theFormation, Ministry, and Life of Permanent Deacons in the United States(ND), the bishops of the United States wish to ensure on the partof all involved a more effective preparation for and post-ordinationsupport of diaconal ministry.As implemented in accord with local or regional resources, thisDirectory will provide a sure directive for promoting harmonyand unity in diaconal formation and ministry throughout theUnited States and its territorial sees. In so doing, this Directorywill ensure certain uniformity in the identity, selection, and formationof deacons, as well as provide for more clearly definedpastoral objectives in diaconal ministries. The diocesan program for diaconal formation and post-ordinationsupport of deacons are under the immediate supervision of thediocesan bishop, his diocesan director of the diaconate, and otherpersonnel to whom the bishop has entrusted areas of responsibility.To assist diocesan supervisory personnel in the on-going process ofimproving diaconal formation and making more effective the postordinationsupport of deacons, the Bishops’ Committee on theDiaconate (BCD) provides this policy statement and appends a suggestedSelf-Study Instrument.C.
