The text <strong>of</strong> <strong>the</strong> December 2004 Sixth Revision <strong>of</strong> <strong>the</strong> Bishops’Committee on <strong>the</strong> Diaconate (BCD) Policy Statement: Visit <strong>of</strong>Consultation Teams to Diocesan Diaconate Programs.2. A Self-Study InstrumentThe Sixth Revision <strong>of</strong> this BCD Policy Statement is based on <strong>the</strong>2004 <strong>National</strong> <strong>Directory</strong> <strong>for</strong> <strong>the</strong> <strong>Formation</strong>, <strong>Ministry</strong>, <strong>and</strong> <strong>Life</strong> <strong>of</strong><strong>Permanent</strong> Deacons in <strong>the</strong> United States.The Policy Statement is provided as a guide <strong>for</strong> a diocesan bishop whowishes <strong>for</strong>mally to request a visit <strong>of</strong> a BCD Consultation Team. Sucha visit is an opportunity <strong>for</strong> dialogue, discussion, <strong>and</strong> consultationwhich is summarized in a Consultation Team report to <strong>the</strong> requestingbishop. The report is confidential in nature. It becomes <strong>the</strong>property <strong>of</strong> <strong>the</strong> requesting bishop to be distributed <strong>and</strong> used at hisdiscretion. It is <strong>of</strong>fered as a service to a bishop <strong>and</strong> those to whomhe has given <strong>the</strong> responsibility <strong>for</strong> <strong>the</strong> <strong>for</strong>mation <strong>of</strong> deacons <strong>and</strong> <strong>the</strong>post-ordination life <strong>and</strong> ministry <strong>of</strong> deacons.The Self-Study Instrument is a guide <strong>for</strong> a diocese to reflect on <strong>the</strong>experience <strong>of</strong> <strong>the</strong> diaconate <strong>and</strong> a self-critique <strong>of</strong> <strong>the</strong> degree <strong>of</strong>implementation <strong>of</strong> <strong>the</strong> provisions <strong>of</strong> <strong>the</strong> United States Conference<strong>of</strong> Catholic Bishops’ 2004 <strong>National</strong> <strong>Directory</strong> <strong>for</strong> <strong>the</strong> <strong>Formation</strong>,<strong>Ministry</strong>, <strong>and</strong> <strong>Life</strong> <strong>of</strong> <strong>Permanent</strong> Deacons in <strong>the</strong> United States. Such aself-study could be implemented at times when <strong>the</strong> need becomesevident or as part <strong>of</strong> <strong>the</strong> preparation <strong>for</strong> a visit by a BCDConsultation Team. In that case, <strong>the</strong> self-study should be accomplishedin advance <strong>of</strong> requesting such a visit. The results should bepart <strong>of</strong> <strong>the</strong> materials provided to <strong>the</strong> Consultation Team as <strong>the</strong>y prepare<strong>for</strong> <strong>the</strong>ir visit. The provisions <strong>of</strong> Appendix I (Specific NormativeElements in Diaconal <strong>Formation</strong>) to <strong>the</strong> <strong>National</strong> <strong>Directory</strong> apply.The members <strong>of</strong> <strong>the</strong> Bishops’ Committee on <strong>the</strong> Diaconate hopethat <strong>the</strong>se documents will help to streng<strong>the</strong>n diocesan programs <strong>of</strong><strong>for</strong>mation <strong>and</strong> post-ordination continuing education <strong>and</strong> spiritual<strong>for</strong>mation <strong>of</strong> deacons.
