Please carefully read the Certification Information Handbook before filling out theRequest for Transcript Evaluation.2. Fill out the Request for Transcript Evaluation completely and return it with copies oftranscripts from ALL schools attended. Student copies of transcripts are acceptable forevaluation purposes.3. If your transcript does not indicate semester or quarter hour credits, or credit isreported in other units, include a key to the transcript.4. Michigan Test for Teacher Certification (Basic Skills and Subject Area Tests) ReportingRequirements. Michigan Test for Teacher Certification (MTTC) examination scoresmust be furnished directly to Wayne State University by Evaluation Systems Group ofPearson, the MTTC testing agency. When registering for the MTTC, select “WayneState University (31)” as a “College or University to Receive Scores.” If yourexamination scores were not released to Wayne State University, please request anoriginal score report from Evaluation Systems Group of Pearson. From the ScoreReport Explanation link on the MTTC website (http://www.mttc.nesinc.com/), selectTest Results Request Form and enter 31 for the Institutional Code to have your scoressent to Wayne State University (31). Your scores must be mailed directly fromEvaluation Systems Group of Pearson to Wayne State University. An original scorereport is required by the Michigan Department of Education for verification of testscores.
