StandingStanding principlesWheelchair users are at high risk of developing secondaryconditions because they often sit for long periodsof time. These secondary conditions can reducetheir quality of life significantly.The advantages of putting users into a standing positioninclude increasing bone mass, strengtheningmuscles, furthering respiration, and enhancing developmentof the hip socket. The upright positionalso increases a user’s viewing area and enhances itsfeeling of well-being.We recommend that disabled children get their firststanding frame at about the same time most childrenstart total weight bearing on their legs, i.e. around theage of one year.When standing:When positioning children in standing frames it is importantto use the pelvis as a key point. The pelvismust be symmetrical and it must be neutrally tilted.Weight bearing is important to the user when developingand strengthening the hip joints.Standing can:• increase bone density• decrease joint contractures• improve cardiopulmonary function• improve digestion• increase bone growth• increase alertnessMany CP patients are users of standing frames. Theysay the spasticity in their legs decreases after regularuse of the standing frame. WalkingWalking principlesTo walk is a skill most people consider as a naturalphenomenon and therefore do not really think about.Learning to walk is a long process, which lasts over3-4 years, starting with few uncontrolled steps endingup as a controlled skill.Walking is the simplest way for a person to move.The whole body is involved, skeleton, joints, muscles,nerves, heart, lungs etc. Disabled children can havedifficulties in walking for various reasons, they musttherefore be guided and stimulated in taking theirfirst steps.process is important to know because this is what weaim for when teaching a child to walk.Start walking:Putting the weight on one leg, leaning forward to unbalancethe body (the centre of gravity falls in frontof the body’s plumb line), swinging the other legforward and placing it on the ground in front of thebody’s centre of gravity (it becomes the supportingleg), lifting the first leg from the floor and swinging itforward...Walking can give a child the opportunity of gettingaround, socializing and interacting with its peers.Walking can:• improve bone density• decrease joint contractures• improve cardiopulmonary function• improve digestion• increase leg muscle strength• decrease spasticity• improve hip development• promote psylogical wellnessIn normal development the child starts crawling at eightmonths, standing at ten months and walking at 12-15months.
