5.09CHECK-OUT / CHECK-IN <strong>POLICY</strong>Check-out Procedures1.In order to leave school during the school day, students must be signed out in theschool office by a custodial parent,* guardian, or other adult who has been properlydesignated in writing by the parent/guardian. The parent/guardian or designated adultmust come in person to the school office. Students will not be permitted to sign outon the basis of a note or phone call from parent(s) or others.2.The School Registration / Information Form should be used by the parent/guardian todesignate adult(s) who may sign the student out of school. Any adult who is identifiedby the parent/guardian as an emergency contact person on the School Registration /Information Form is deemed authorized to sign the student out of school. Bydesignating such person(s), the parent/guardian is authorizing the school torelease the student at any time a designated person comes to the school office tosign the student out. Nevertheless, administrators may exercise discretion to contactthe parent/guardian as deemed appropriate or necessary.3.Except for periodic updates which may be requested by the school, a parent/guardiandesiring to update or change the listing of persons designated to sign the student out ofschool should visit the school office. Such updates or changes shall be theresponsibility of the parent/guardian.4.If a high school student is to be allowed to leave the campus for an appointmentwithout a parent/guardian or designated adult present, the parent/guardian must comeby the school office in advance of the check-out time (i.e., before school or on the daybefore the desired check-out time) to sign the check-out sheet or other pre-release formin the presence of school personnel.5. The Attendance Policy shall be applied to determine whether absences fromindividual classes are excused or unexcused and whether the student is eligible tomake up work. (Thus, any absence from class beyond the fifth (5th) in a semester willrequire a doctor's excuse or parent conference with the principal for consideration ofexcused status and eligibility to make up work.)6. Students who leave campus without following the check-out procedure will becharged with an Unauthorized Absence (Intermediate Violation) under the Code ofStudent Conduct.7.Excessive check-outs are strongly discouraged by the Board, as are check-outsduring the final hour of school unless absolutely necessary. After four (4) early checkoutswithin a semester, the principal's permission may be required for additional checkouts.
