Being completely independent our clinicians source components based on individual needs, oftenthinking ‘outside the box’ in solving more complex cases.ConsultationAn initial consultation with one of our consultant Prosthetists lasts approximately 1 hour and will covertopics such as medical history, previous prescription, your expectations and discussing the way forward.A quotation is sent out shortly after your consultation, giving details of costs and prescriptions for theprosthesis discussed.AppointmentsFollowing the acceptance of your quotation, a detailed schedule of appointments is made incorporating thefollowing:• Casting• Trial socket fitting – this may involve more than one appointment to ensure complete comfort• Fitting – the definitive socket used for your prosthesis• Shaping – Prosthetist shapes the foam by handA further two appointments are necessary for a silicone cosmesis. Basics to bionics...Types of ProsthesisPROSTHETICSAt <strong>Dorset</strong> <strong>Orthopaedic</strong>, we realise that everyone is an individual withdifferent needs. We offer various types of prostheses according to the levelof amputation, which provide varying functions. Our foremost concern is thecomfort of the socket, then we consider the function of the limb and finally thetype of finish required.Upper Extremity ProsthesisOur clinicians are qualified and experienced in the fitting of all types of upperextremity prostheses, from individual digits to full shoulder length prostheses.We regularly fit body powered and myo-electric functional limbs, as well ascustom made high definition silicone cosmetic arms and hands. Choosing the finish for the prosthesis...There are various types of finishes available to coveryour prosthesis. This is very much a personal choiceand would depend on the function that is required.CoverlessSometimes the prosthesis requires no cover atall. Limbs used for high levels of activity, such asrunning, would need to be lighter and thus no coveris used. Quite often, clients like the look of thecarbon componentry and decide to opt for no cover.PROSTHETICSSuperskin FinishA flexible spray finish which can be matched to yourskin tone using various coloured sprays. Why silicone...For many people a silicone cosmesis completes the pictureand provides additional confidence. Clients often describeit as a liberating experience. The focus returns to the client,rather than the prosthesis or the need to conceal it.Silicone in its uncured state can be sculpted with greataccuracy. Once cured, the silicone remains flexible andprovides a truly life-like and durable appearance.All our work starts with the client. Partial Hands & DigitsSilicone is particularly suited to working with partiallimbs.Built on a cast of the client’s residual limb,technicians are able to work with different types ofsilicone and soft gel inserts to create a comfortablefull or partial cosmesis with breath-taking levels ofdetail.SILICONEPartial Feet & ToesMany partial feet include gel pads for protectionof sensitive areas. Depending on the prescriptionrequired, carbon blades can be embedded withinthe silicone to help with function. As with all oursilicone work, detailing and finish are fully bespoke.Cosmetic RestorationCosmetic restoration is an important andspecialised area within silicone. Individuals who, formany years have endured unbalanced limb shapesdue to disease or accidents causing tissue loss cannow be fitted with individually specified and madesilicone covers.Skin colouring, shape and your own individualcharacteristics are sculpted into the cosmesis and,where necessary, soft gels can be enclosed withinthe silicone to prove greater levels of comfort andprotection.dorset orthopaedic 13


What our clients say...Neil HeritageIn November 2004, whilst serving withan army bomb disposal unit, CorporalNeil Heritage of the Royal Signals wasinjured in Iraq. Neil had both his legsamputated above the knee and wastold he would probably never walkagain.“I have been an amputee for justover 6 years and have always hada problem with my right stump. Itdecreases in volume throughout theday meaning my prosthesis would falloff after a couple of hours use.I first met Bob Watts over a year ago,not to talk about my existing walkinglimbs but for Bob to fit me with a pairof running legs so I could achieveone of my goals of competing in a Triathalon.After our first meeting, <strong>Dorset</strong><strong>Orthopaedic</strong> kindly offered to sponsorme to compete in the event. Betterstill, Bob had a solution for my socketproblems and I was fitted with thenew Lok-In liner socket system. Thesemade a big difference as previousoff the shelf liners had not been longenough to cover the top part of my leg,which led to sores where the socketsrubbed my skin.Another difference I noticed beingtreated at <strong>Dorset</strong> <strong>Orthopaedic</strong> was theamount of time Bob spent with me,especially at the check socket phase.He kept making tiny changes until thesockets where perfect.When the sockets were complete,I met with <strong>Dorset</strong> <strong>Orthopaedic</strong>’sPhysiotherapist, Justin Scriven.Justin gave me an examination,some exercises to practice at homeand spent a few sessions with meimproving my running technique.I was able to run 3-4 times a week,building up my fitness and I wassurprised at how long I could use mylimbs for, at such high impact, withoutfeeling any pain in my stumps fromthe sockets. I found I was also ableto use my running limbs for a rangeof other activities including circuitclasses and boxing.Now my walking limbs didn’t meet thesame standard of my running limbs;I was able to run for longer than Icould walk! Being a former soldier, Iwas very fortunate as soldier servicecharities helped me by funding apair of Lok-In sockets for my walkinglimbs. Since having these sockets Ihave achieved my long term goal of nolonger using a wheelchair. I wear mylegs for up to 15 hours a day and havestill had no pain or discomfort. He workedat the Royal Bournemouth Hospital before workingoverseas in Australia and New Zealand. Orthotic clinic...Orthotic is the term derived from the Greek ‘ortho’,meaning straight or normal and ‘tics’, meaningsystematic pursuit. Literally, orthotics is thestraightening, supporting and correcting of limbs orthe spine using splints, braces and supports.<strong>Dorset</strong> <strong>Orthopaedic</strong> have been supplying a privateOrthotic service since the official opening of ourfirst Orthotic clinic in Ringwood, Hampshire, in2001. This service now extends to our clinics inBurton upon Trent and London. Our Orthotistsdeal with patients suffering with a multitude ofconditions including spinal injuries, peripheralneuropathy, MS, Polio, Stroke, Cerebral Palsy andsports injuries to name but a few.Our Orthotists are experienced in prescribing andfitting all types of traditional and complex orthoticdevices; our services now include the prescriptionof special seating and wheelchairs/adaptationswhere required. Silicone Ankle Foot OrthosisSAFO...The SAFO (Silicone Ankle Foot Orthosis),designed by <strong>Dorset</strong> <strong>Orthopaedic</strong>, is an awardwinning revolutionary ankle foot orthosis.Designed as an alternative to the more rigidstyle of traditional AFOs, the SAFO is nowbeing extensively used in the treatment ofmany conditions that typically result in partialor complete loss of active range of movementat the ankle/foot.The SAFO provides support down the front ofthe leg and on to the top of the foot to assistthe lifting of the foot during the swing phaseof walking, thus preventing the patient fromtripping or walking on their toes.Medical trials show that the SAFO alleviatessymptoms of drop foot that affects manyCharcot Marie Tooth (CMT), Polio, CerebralPalsy, MS and stroke sufferers worldwide.The clinic has also successfully fittednumerous other conditions that result in whatis commonly referred to as ‘drop foot’.ORTHOTICS“The SAFO has turned my lifearound. What our clients say...Matt Mbanga“April’s sultry advance seems tounofficially mark the end of the rainshere in Zimbabwe. The dreaded coolweather of our highland wintershas not yet arrived though the rainhas all but gone - leaving us with ahot, dusty, dry and for rural farmers,very uncomfortable position for themonth of the harvest.I have not noticed any noticeablechange in the comfort of mySAFO as the season changes. Ispend most of my time around theproject switching between rubber‘Wellington’ boots in the fieldsand trainers around the seedlingsand herb beds. Both sets of shoesoffer plenty of stability and theSAFO allows me to move aroundand carry out the necessary taskswithout any discomfort. I usedto strap an elasticised bandagearound my ankle and pull it tightenough to raise my toes and holdmy foot steady enough to not tripover as I walked about. I was alwaysconscious of my foot and learnedto live with the discomfort after 6years. I used to have to unwrap andmassage a very constricted anduncomfortable foot at least everytwo hours before re-strapping it tocarry on.
