Summary - Peripheral Neuropathy as a Risk Factor• Prospective studies show a strong relationship between the presence <strong>of</strong> peripheralneuropathy (assessed by a biothesiometer or Semmes-We<strong>in</strong>ste<strong>in</strong> mon<strong>of</strong>ilament)<strong>and</strong> subsequent ulceration• There is a similarly strong relationship between peripheral neuropathy <strong>and</strong>subsequent amputation• Cross-sectional studies have consistently demonstrated that peripheral neuropathyis an <strong>in</strong>dependent risk factor for foot ulceration <strong>and</strong> amputation <strong>in</strong> people withdiabetes• The presence <strong>of</strong> peripheral neuropathy requires other permissive factors <strong>and</strong>/or aseries <strong>of</strong> events (some <strong>of</strong> which may be preventable or reversible) to culm<strong>in</strong>ate <strong>in</strong>an ulcer or amputation• More research is required to clarify the factors which <strong>in</strong>fluence the differentpathways from peripheral neuropathy to ulceration <strong>and</strong> amputation18
