CRESTVIEW LOCAL SCHOOLS HONORED<br />
<strong>Crestview</strong> <strong>Local</strong> <strong>School</strong>s has been named one of the most cost-effective school<br />
districts in the state when it comes to providing administrative services.<br />
<strong>The</strong> district was presented a certificate of achievement for efficiency by Ohio<br />
Education Matters, a public policy research organization.<br />
―<strong>The</strong> taxpayers and residents of this community should be proud that their district<br />
takes seriously the best use of resources to benefit children,‖ said Andrew Benson,<br />
executive director of Ohio Education Matters.<br />
<strong>The</strong> recognition <strong>Crestview</strong> received stems from a report issued by the organiza-<br />
tion this year as part of a study of K to 12 education called Ohio Smart <strong>School</strong>s. Of<br />
the more than 600 districts in the state, the study found <strong>Crestview</strong> was one of four<br />
districts to be among the most efficient in both school-level administration and central<br />
–office administration. Treasurer Tina Bennett said the district was pleasantly<br />
surprised by the recognition. ―We’re always looking for ways to cut costs,<br />
Bennett said. ―Reductions in non-instructional areas we feel are preferable to areas<br />

CRESTVIEW SPORTS PHYSICALS<br />
FOR 2011-12 SCHOOL YEAR<br />
AND ATHLETIC NEWS<br />
Please Note: Physicals will be performed at <strong>Crestview</strong> High <strong>School</strong> on Tuesday, July<br />
26, 2011. Physicals for the middle school students will begin at 6:00 PM and physicals<br />
for the high school students will begin at 6:30 PM You must complete the OHSSA<br />
Pre-Participation Physical Exam Forms packet (located in the high school or middle<br />
school offices or on the <strong>Crestview</strong>’s website under the Athletics tab. Make sure it is<br />
filled out completely and signed by your parent/guardian. Cost is $10.00.<br />

July 26, 2011 Physical Night at <strong>Crestview</strong> High <strong>School</strong><br />
6:00 PM for MS and 6:30 PM for HS students. $10 fee.<br />

Athletic Forms & Athletic Fee:<br />
All athletic forms (Emergency Medical Form, Athletic Handbook Acknowledge Form,<br />
and Medical Insurance Coverage Form) need to be turned in to your coach the first week<br />
of practice. <strong>The</strong>se forms will be available during the Physical Night and during the<br />
Parent Information Night. <strong>The</strong> $35.00 Athletic Fee must be paid to the athletic director<br />
or athletic secretary by the first scheduled sporting event.<br />

CAFETERIA NEWS<br />
Elementary Lunch Price Increase<br />
Due to requirements in the Healthy,<br />
Hunger-Free Kids Act of 2010, we<br />
will be raising the price of an Elementary<br />
lunch from $2.25 to $2.30 for the<br />
2011-2012 school year. Lunch prices<br />
in the Middle & High <strong>School</strong>s will<br />
remain at $2.50.<br />
EZpay – Last year, many parents<br />
took advantage of paying for school<br />
fees and lunches by using EZpay, our<br />
on-line payment system. All you need<br />
is your student’s 6-digit ID number,<br />
click on the EZpay link on the<br />
C r e s t v i e w w e b s i t e a t<br />
(www.crestviewschools.net) and you<br />
can set up an account. Even if you<br />
don’t pay for lunches or school fees<br />
using EZpay, you can still set up an<br />
EZpay account to view your student’s<br />
lunch account balance or view their<br />
lunch history. You can obtain your<br />
student’s 6-digit ID number by contacting<br />
the Food Service department at<br />
419-895-1700, ext. 5008 or by calling<br />
the secretary in your student’s building.<br />
NOTE: <strong>The</strong>re is a convenience<br />
charge of $2.00 each time you use the<br />
EZpay system to make a payment.<br />
Charging Lunches – Our guidelines<br />
for lunch charges will remain the<br />
same. Only a complete lunch may be<br />
charged – no ala carte items may be<br />
charged. Students in the Elementary<br />
are allowed three charges; Middle<br />
<strong>School</strong> two charges; and High <strong>School</strong><br />
one charge. When a charge is paid, a<br />
student is then eligible to charge<br />
again.<br />
Free & Reduced Meals – Free and<br />
Reduced applications will be sent<br />
home with your children on the first<br />
day of school. <strong>The</strong>y should be available<br />
on the school web site in early<br />
August. If you are direct certified,<br />
you will receive a letter from the Food<br />
Service Department during August<br />
and you will not need to fill out an<br />
application even though one will still<br />
come home with your student(s).
