A Friendly Reminder for Parents “Are You Up-to-Date on your Immunizations?” FOR 2011-12 SCHOOL YEAR! A reminder that students beginning 7 th grade in the fall of 2011 will be required to have a Tdap (Tetanus, diphtheria and pertussis) vaccine before the start of the school year. This is a new state of Ohio vaccine requirement. The vaccine contains protection against pertussis, or whooping cough, a severe respiratory illness that causes violent rapid coughing that can last for extended periods of time. It is spread by coughing and sneezing. Cases of whooping cough mostly occur in school settings and nearly doubled in Ohio between 2008 and 2009. An updated immunization record must be turned in to the middle school office by August 24, 2011, in order for a student to be eligible to attend school. Any 7 th grade student who does not have proof of this immunization by September 13, 2011, will be excluded from school until we have an updated immunization record. For special exemptions please refer to ORC 3313.671. The summer months will provide ample time for parents to get their children’s immunizations up-to-date. We encourage you to contact your personal physician, the Ashland County Health Department (419-282-4317) or the Mansfield-Ontario-Richland County Health Department (419-774-4540) to set up an appointment or get the times of clinic hours. The Crestview Middle School office will be open Monday through Friday until June 24, 2011 and re-open on August 1, 2011. Also, you may drop off your child’s updated immunization record at the Crestview Board of Education office. Summer office hours are Monday-Thursday from 7:00 AM to 4:30 PM and on Friday 7:00 AM - 11:00 AM. HS Report Cards And Schedule Pick-Up Dates Final high school report cards can be picked up starting on June 13, 2011, during summer office hours: Monday—Thurs. 7:00 AM—4:30 PM Friday 7-11:00 AM 2011-12 Class Schedules can be picked up starting on August 18, 2011 during Meet-The -Teams and Open House starting at 6:00 PM. Schedules may also be picked up during regular school office hours. 2011 Prom A Red Carpet Afair The 2011 Prom was a huge success with 140 attending. The King and Queen were: Zach Howell and Sarah Jenkins. Prince and Princess were: Makaylee Crank and Garrett Prise. Congratulations to these students and many thanks to all the hands that helped put together this year’s Prom and After Prom. Crestview FFA and Alumni The Crestview FFA and FFA Alumni are seeking volunteers to work the FFA Steak Trailer and the Firelands Farmer booth during the Richland County Fair August 7-13, 2011. V O L U M E 2 0 1 0 - 1 1 , I S S U E 6 Community & General Information CRESTVIEW LOCAL SCHOOLS HONORED Crestview Local Schools has been named one of the most cost-effective school districts in the state when it comes to providing administrative services. The district was presented a certificate of achievement for efficiency by Ohio Education Matters, a public policy research organization. ―The taxpayers and residents of this community should be proud that their district takes seriously the best use of resources to benefit children,‖ said Andrew Benson, executive director of Ohio Education Matters. The recognition Crestview received stems from a report issued by the organization this year as part of a study of K to 12 education called Ohio Smart Schools. Of the more than 600 districts in the state, the study found Crestview was one of four districts to be among the most efficient in both school-level administration and central –office administration. Treasurer Tina Bennett said the district was pleasantly surprised by the recognition. ―We’re always looking for ways to cut costs, Bennett said. ―Reductions in non-instructional areas we feel are preferable to areas that are going to affect students in the classroom.‖ - Written by Bryan Bullock, Mansfield News Journal, May 19, 2011. CRESTVIEW SPORTS PHYSICALS FOR 2011-12 SCHOOL YEAR AND ATHLETIC NEWS Please Note: Physicals will be performed at Crestview High School on Tuesday, July 26, 2011. Physicals for the middle school students will begin at 6:00 PM and physicals for the high school students will begin at 6:30 PM You must complete the OHSSA Pre-Participation Physical Exam Forms packet (located in the high school or middle school offices or on the Crestview’s website under the Athletics tab. Make sure it is filled out completely and signed by your parent/guardian. Cost is $10.00. Upcoming Important Athletic Dates: July 26, 2011 Physical Night at Crestview High School 6:00 PM for MS and 6:30 PM for HS students. $10 fee. Checks made out to Crestview High School July 27, 2011 7:00 PM—Parent Information Night. Mandatory meeting for HS & MS Fall sports athletes and their parents. HS Aud. August 1, 2011 Official practice begins for high school football and golf. August 8, 2011 Official practice begins for all other HS/MS fall sports Athletic Forms & Athletic Fee: All athletic forms (Emergency Medical Form, Athletic Handbook Acknowledge Form, and Medical Insurance Coverage Form) need to be turned in to your coach the first week of practice. These forms will be available during the Physical Night and during the Parent Information Night. The $35.00 Athletic Fee must be paid to the athletic director or athletic secretary by the first scheduled sporting event. P A G E 7 CAFETERIA NEWS Elementary Lunch Price Increase Due to requirements in the Healthy, Hunger-Free Kids Act of 2010, we will be raising the price of an Elementary lunch from $2.25 to $2.30 for the 2011-2012 school year. Lunch prices in the Middle & High Schools will remain at $2.50. EZpay – Last year, many parents took advantage of paying for school fees and lunches by using EZpay, our on-line payment system. All you need is your student’s 6-digit ID number, click on the EZpay link on the C r e s t v i e w w e b s i t e a t (www.crestviewschools.net) and you can set up an account. Even if you don’t pay for lunches or school fees using EZpay, you can still set up an EZpay account to view your student’s lunch account balance or view their lunch history. You can obtain your student’s 6-digit ID number by contacting the Food Service department at 419-895-1700, ext. 5008 or by calling the secretary in your student’s building. NOTE: There is a convenience charge of $2.00 each time you use the EZpay system to make a payment. Charging Lunches – Our guidelines for lunch charges will remain the same. Only a complete lunch may be charged – no ala carte items may be charged. Students in the Elementary are allowed three charges; Middle School two charges; and High School one charge. When a charge is paid, a student is then eligible to charge again. Free & Reduced Meals – Free and Reduced applications will be sent home with your children on the first day of school. They should be available on the school web site in early August. If you are direct certified, you will receive a letter from the Food Service Department during August and you will not need to fill out an application even though one will still come home with your student(s).
