<strong>VISA</strong> <strong>FOR</strong> <strong>LOSS</strong> <strong>AND</strong> <strong>THEFT</strong> <strong>OF</strong> <strong>DOCUMENTS</strong>This visa is applied for when a legal resident in Spain, who is temporarily in India,Nepal, Bhutan, Sri Lanka or the Maldives reports loss or theft of his/her residence card,hence impeding his/her return to Spain.It is important to note that this visa requires consultations with the correspondingauthorities in Spain. This can take up to various weeks.After the submission of the application, the applicant can be requested furtherdocumentation or personal interview.Forging of documents and impersonation of identity will be immediately reported to theSpanish and the Indian police authorities and the applicant will be liable to actionagainst him/her.The following documents are essential for a complete visa application.Application Form: Application for Schengen Visa.The application must be submitted personally by the applicant at the Embassy of Spainin New Delhi or Consulate General of Spain in Mumbai, strictly in accordance with thecurrent place of residence of the applicant. Applications sent by courier or by post arenot entertained.The fee must be paid via bank draft payable at New Delhi/Mumbai in favor ofEMBASSY <strong>OF</strong> SPAIN, NEW DELHI or CONSULATE GENERAL <strong>OF</strong> SPAIN, MUMBAI,in accordance with the place of application. The applicant must write his/her name, dateof birth, and passport number behind the draft. A photocopy of the draft must besubmitted along with the application.Passport of the applicant which should be valid for at least four months at the time ofapplication. Photocopy of all the pages of the previous (if any) and present passportmust be submitted.


Original Application form for Schengen Visa duly filled in and signed by the applicant inblue ball point ink. A recent photograph (35-45 mm) of the applicant with whitebackground must be attached in the application form. The application form must becorrectly filled, facilitating all the possible information requested in the form includingpostal address, email address and telephone number.In case of minors (below 18 years), the application form must be signed by either orboth the parents. Photocopies of passports of both the parents, attested by notarypublic, must be attached.Two photocopies of the application form along with two recent photographs (35-45 mm)of the applicant with white background.Detailed covering letter from the applicant, mentioning the date of departure from Spainand the date of entry into the country of current residence.Original Police information: First Information Report (FIR) in English reporting the lossor theft of the residence card of the applicant.Photocopy of the residence card of the applicant.Photocopy of other documents (if there be any) as proof of residence in Spain.Extra recent photograph (35-45mm) of the applicant with white background.Please note that this is not an exhaustive list. If necessary, you may be requested to bringadditional documentation or to appear for a personal interview.
