This impressive staff strength and this ideal pyramid of supervisory staff to manage thestaff would be the envy of every programme manager <strong>in</strong> the health system. It is to thecredit of the <strong>EMRI</strong> that it could put <strong>in</strong> place such a structure. It is unlikely that any otherprogramme, especially if done with<strong>in</strong> the <strong>gov</strong>ernment would have been allowed this. Andyet this adequacy <strong>in</strong> the supervisory structure is essential to this schemes success.When costs are lowered through a tender<strong>in</strong>g process, or there is a rationalisation ofcosts that is attempted, it is precisely this part that may get axed, and that would not begood for the outcomes.f. Salary structures have been a matter of discussion. The operational staff are the pilots,the EMT (paramedical staff), and the call operators. At the levels of paramedicalworkers, one concern has been that the higher salaries may be lead<strong>in</strong>g to shift<strong>in</strong>g ofhuman resources with<strong>in</strong> the states, lead<strong>in</strong>g to further pressure on public sector posts.However the need for higher skill levels, work schedule of 12-hour a day (half of whichare as night duties), and a more <strong>in</strong>tensive nature of work are sufficient reasons for thehigher payment. The EMT (emergency medical technician), a graduate, gets 1.00 lakhsper year or about Rs. 8000 per month and the driver - about Rs.0.75 lakhs per year orRs. 6000 per month. Their immediate supervisors are the operations executive and thefleet executive and they would get monthly Rs. 18,000 to Rs. 25,000 monthly. Thedistrict level managers get about Rs. 40,000 per month. The state level managers wouldcommand about Rs 6. lakhs per year – and all this is as cost to company. This much ispart of what is now shown as operational costs. The audit report of Rajasthan <strong>in</strong>dicatesthat policies <strong>in</strong> pay fixation for middle level managers are sub-optimal lead<strong>in</strong>g to wagejumps for new recruits at higher and middle level management levels, but this is difficultto comment on and anyway relates to very few posts. Though such structures could beexam<strong>in</strong>ed for further economies, most of it is likely to rema<strong>in</strong>.g. Dur<strong>in</strong>g discussions it was expla<strong>in</strong>ed that <strong>in</strong> addition to the above mentioned operationalstaff levels, there are four levels of leadership, what <strong>EMRI</strong> refers to as the leadershiplevels. Of these there is a total of 200 staff, of which 100 at the national level and aboutanother 100 at the rate of about 10 per state. We were told by the <strong>EMRI</strong> officials that forthese leadership levels, the annual salary ranges from Rs.12 lakhs for the lowest <strong>in</strong> thisgroup to Rs.36 lakhs for the state head of operations, averag<strong>in</strong>g at Rs 18 lakhs per year.This comes to a recurrent bill of Rs 36 crores per year that no one is pay<strong>in</strong>g for currentlyand for which <strong>EMRI</strong> <strong>in</strong>tends to f<strong>in</strong>d <strong>in</strong>dependent sources. There was an understand<strong>in</strong>gamongst some stakeholders that this could be seen as the 5% operational contributionthat <strong>EMRI</strong> makes - but <strong>EMRI</strong> specifically denied this and asked for this “leadershipcosts” to be seen as seen as over and above the 5% “operational costs contribution”.The latter, <strong>EMRI</strong> is anyway not <strong>in</strong> a position to pay.
