Study of Emergency Response Service (<strong>EMRI</strong> Scheme)Compar<strong>in</strong>g these audited statements of expenditures and the daily reports of ambulanceusage, each ambulance is averag<strong>in</strong>g 8.1 trips per day, thus result<strong>in</strong>g <strong>in</strong> an operat<strong>in</strong>g costper ambulance of Rs. 565 13 per trip per day, or Rs. 12.59 lakhs per year, approximately.These figures are based on <strong>EMRI</strong> reports. Payments are made on the basis of operat<strong>in</strong>gcosts as presented periodically. There is no process of <strong>in</strong>dependent or third-party validationis <strong>in</strong> place <strong>in</strong> the state. Look<strong>in</strong>g at the break-up of operat<strong>in</strong>g cost, we f<strong>in</strong>d that <strong>in</strong> AndhraPradesh, salary (of operations staff) consumes 27% of the opex, adm<strong>in</strong>istrative expensesare 31% and cost of runn<strong>in</strong>g the ambulances is only 41% of the opex. This is when theambulance usage has stabilised at around 8 trips per ambulance per day.In Andhra Pradesh, there is another service for remote medical care where<strong>in</strong> a person candial 104 and avail of free medical advice from health care professionals. This is managed byanother body called the Health Management and Research Institute (HMRI), which was alsopromoted by Shri Raju, and was possibly l<strong>in</strong>ked to Satyam. Calls to 104 are prioritized andcallers routed to appropriate dest<strong>in</strong>ations, <strong>in</strong>clud<strong>in</strong>g 108 if an ambulance is required. Though<strong>in</strong> name and <strong>in</strong> basic design there are parallels, the HMRI operations are quite <strong>in</strong>dependentof the <strong>EMRI</strong> and would need to be evaluated <strong>in</strong>dependently. Given an <strong>in</strong>vestment of overRs.250 crores <strong>in</strong> the HMRI, this too would need an <strong>in</strong>dependent evaluation.2. <strong>EMRI</strong> <strong>in</strong> Gujarat<strong>EMRI</strong> began operations <strong>in</strong> Gujarat with the sign<strong>in</strong>g of the MOU with Government of Gujaratand launch<strong>in</strong>g of operations on August 29, 2007 with a fleet of 61 (20 ALS and 41 BLS)ambulances across 42 towns (<strong>in</strong> 9 districts) of the state. The capital cost for purchase andequipp<strong>in</strong>g the ambulances as well as for land and build<strong>in</strong>g of the call centre, was providedby the Govt. of Gujarat (GoG) under NRHM. Under the MOU the GIS based software forprovid<strong>in</strong>g the back-end support from the Call Centre was to be commissioned andma<strong>in</strong>ta<strong>in</strong>ed by <strong>EMRI</strong>. <strong>EMRI</strong> was responsible for handl<strong>in</strong>g medical, police and fireemergencies through the 108 Emergency Service. In the subsequent year 2008, theoperations were extended to the entire state with 380 (33 ALS and 347 BLS) ambulancesacross 319 cities <strong>in</strong> all the 26 districts 14 . The February 17 th report (Annexure A-16) showsthe number of ambulances as 402.The Govt. of Gujarat is contribut<strong>in</strong>g 95% of the operat<strong>in</strong>g cost and 100% of the capital cost(of ambulance purchase, fitt<strong>in</strong>gs, land and build<strong>in</strong>g for the state level Call Centre). The Govt.of Gujarat’s share of the cost is charged under the NRHM.
