Precursors to accidents<br />

‘Precursors to accidents’ refers to <strong>in</strong>dicators measur<strong>in</strong>g <strong>in</strong>cidents that under o<strong>the</strong>r circumstances would have led to an accident. There<br />

are <strong>in</strong>dicators for broken rails, track buckles, signals passed at danger, wrong-side signall<strong>in</strong>g failures, broken wheels and broken axles.<br />

The reported number of track buckles has reduced from 7 803 <strong>in</strong> 2006 to 835 <strong>in</strong><br />

2008, and <strong>the</strong> reported number of signals passed at danger (SPADs) has <strong>in</strong>creased<br />

threefold. The differences are ma<strong>in</strong>ly accounted for by Italy and Poland which<br />

have changed def<strong>in</strong>itions dur<strong>in</strong>g <strong>the</strong> period, as is apparent <strong>in</strong> Table 1.<br />

Follow<strong>in</strong>g <strong>the</strong> accident <strong>in</strong> Viareggio <strong>in</strong> Italy <strong>in</strong> 2009, <strong>the</strong> Agency has established<br />

a task force on freight wagon ma<strong>in</strong>tenance <strong>in</strong> cooperation with <strong>the</strong> Member<br />

States. Among <strong>the</strong> objectives of <strong>the</strong> task force are <strong>the</strong> review, exchange and<br />

analysis of <strong>in</strong>formation relat<strong>in</strong>g to problems with broken axles/fatigue and<br />

relevant test<strong>in</strong>g methods, <strong>the</strong> proposal or development of appropriate controls<br />

and monitor<strong>in</strong>g tools, <strong>the</strong> proposal of measures to review <strong>the</strong> different ma<strong>in</strong>tenance regimes exist<strong>in</strong>g across Europe and <strong>the</strong> draw<strong>in</strong>g<br />

up of a programme for fur<strong>the</strong>r harmonisation. With<strong>in</strong> this scope, <strong>the</strong> Agency has also conducted a survey among <strong>the</strong> NSAs on <strong>the</strong><br />

number of broken axles and wheels. The aim of <strong>the</strong> survey was to review <strong>the</strong> figures reported to <strong>the</strong> Agency and <strong>the</strong> def<strong>in</strong>itions used.<br />

The survey showed that most NSAs only reported cracks <strong>in</strong> wheels and axles that led to an accident. Only one country, Germany,<br />

<strong>in</strong>cluded cracks detected dur<strong>in</strong>g regular ma<strong>in</strong>tenance, which is <strong>in</strong> accordance with <strong>the</strong> guidance for <strong>the</strong> revised Annex I to <strong>the</strong> <strong>Railway</strong><br />

<strong>Safety</strong> Directive. Germany reported three cracks that led to accidents <strong>in</strong> 2008, and a total of 752 cracks. This means that <strong>the</strong> figures<br />

reported for broken wheels and axles, displayed <strong>in</strong> Chart 8, only show a part of <strong>the</strong> situation. The survey also showed that problems<br />

with hot boxes are of more concern than cracks <strong>in</strong> wheels and axles.<br />
