Use this <strong>form</strong> to apply for any of the equipment reb<strong>at</strong>es listed below.<br />

2013<br />

Reb<strong>at</strong>e reserv<strong>at</strong>ion required before submitting this reb<strong>at</strong>e applic<strong>at</strong>ion.<br />

Follow these steps to particip<strong>at</strong>e<br />

1. Consult a qualified he<strong>at</strong>ing contractor or plumber to obtain an estim<strong>at</strong>e.<br />

2. Go to www.smart<strong>energy</strong>-zone.com/n<strong>at</strong>ionalgridny to view available funds and to complete the online reserv<strong>at</strong>ion <strong>form</strong>. Customers who do not<br />

have online access must call 1-877-316-9491 to reserve funding. This will reserve funding for your anticip<strong>at</strong>ed reb<strong>at</strong>e. You will receive confirm<strong>at</strong>ion of<br />

your reserv<strong>at</strong>ion via a Web Submission ID. (In order to receive a reb<strong>at</strong>e it is required th<strong>at</strong> you obtain a Web Submission ID by 12/31/13).<br />

3. Complete this applic<strong>at</strong>ion and mail it in within 60 days of reb<strong>at</strong>e reserv<strong>at</strong>ion with:<br />

a) A copy of the equipment’s Air-Conditioning, He<strong>at</strong>ing and Refriger<strong>at</strong>ion Institute (AHRI) certific<strong>at</strong>e (if applying for a boiler or a furnace reb<strong>at</strong>e).<br />

Download a certific<strong>at</strong>e from www.ahridirectory.org or ask your contractor for one.<br />

b) A copy of your d<strong>at</strong>ed invoice.<br />

2013<br />

Terms and Conditions<br />

1. Reb<strong>at</strong>es - Subject to these Terms and Conditions, this program is offered by KeySpan Gas d/b/a N<strong>at</strong>ional <strong>Grid</strong> (“the Company” or “N<strong>at</strong>ional <strong>Grid</strong>”).<br />

The Company, through its contractual vendor, Helgeson Enterprises (the “Vendor”), will pay reb<strong>at</strong>es to eligible customers in the Company’s Service<br />

Territory, for the purchase and install<strong>at</strong>ion of the equipment/products described in this liter<strong>at</strong>ure and within this applic<strong>at</strong>ion.<br />

2. Customer Eligibility - Reb<strong>at</strong>e programs are available to existing N<strong>at</strong>ional <strong>Grid</strong> <strong>gas</strong> he<strong>at</strong>ing customers and customers th<strong>at</strong> convert from oil to <strong>gas</strong> he<strong>at</strong>.<br />

Long Island offer available to customers th<strong>at</strong> reside in Nassau/Suffolk counties and the Rockaway Peninsula where n<strong>at</strong>ural <strong>gas</strong> is available. Some<br />

restrictions may apply. Reb<strong>at</strong>e offers are subject to change without notice and the program may be termin<strong>at</strong>ed <strong>at</strong> any time due to funding constraints.<br />

3. Program Length - New equipment installed from 1/1/13 to 12/31/13 with a reb<strong>at</strong>e applic<strong>at</strong>ion postmarked by 12/31/13 is eligible contingent upon<br />

availability of funds. It must have been new equipment installed <strong>at</strong> the install<strong>at</strong>ion address listed on this applic<strong>at</strong>ion and the required document<strong>at</strong>ion<br />

listed must be postmarked within 60 days of reb<strong>at</strong>e reserv<strong>at</strong>ion request or by 12/31/13, whichever comes first. It is required th<strong>at</strong> you reserve your<br />

reb<strong>at</strong>e funding using the online reb<strong>at</strong>e reserv<strong>at</strong>ion system <strong>at</strong> www.n<strong>at</strong>ionalgridus.com/<strong>energy</strong>efficiencyservices by 12/31/13. Residential customers th<strong>at</strong><br />

do not have online access must call 1-877-316-9491 to obtain a Web Submission ID number.<br />

4. Post-Install<strong>at</strong>ion Work Verific<strong>at</strong>ion - The Company reserves the right to per<strong>form</strong> a verific<strong>at</strong>ion of the specified install<strong>at</strong>ion. If the Company and/or<br />

Vendor determines th<strong>at</strong> the equipment was not installed in a manner th<strong>at</strong> is consistent with program guidelines, the Company and/or Vendor may<br />

require th<strong>at</strong> the install<strong>at</strong>ion be modified before making any reb<strong>at</strong>e payments. The cost of such modific<strong>at</strong>ions is the responsibility of the customer.<br />

5. Energy Use In<strong>form</strong><strong>at</strong>ion - I hereby authorize the Company to release my <strong>energy</strong> use in<strong>form</strong><strong>at</strong>ion to Energy Efficiency Program Sponsors (EEPS) and<br />

System Benefit Charge (SBC) program administr<strong>at</strong>ors and/or its designee. I understand th<strong>at</strong> such in<strong>form</strong><strong>at</strong>ion will be kept confidential and used only for<br />

the purpose of determining program eligibility and <strong>energy</strong> savings.<br />

6. Install<strong>at</strong>ion Requirements - All install<strong>at</strong>ions of he<strong>at</strong>ing and w<strong>at</strong>er he<strong>at</strong>ing equipment and boiler reset controls must be completed in con<strong>form</strong>ance with<br />

st<strong>at</strong>e and local code requirements by qualified contractors.<br />

7. Proof of Proper Install<strong>at</strong>ion - As part of his/her applic<strong>at</strong>ion, the Customer is required to submit d<strong>at</strong>ed copies of all paid invoices documenting the<br />

install<strong>at</strong>ion (including all m<strong>at</strong>erials, labor and equipment invoices) which reflect the actual cost of purchasing and installing the equipment. If label is<br />

unavailable, manufacturer specific<strong>at</strong>ion sheet with model number m<strong>at</strong>ching invoice may be substituted.<br />

8. Indemnific<strong>at</strong>ion - Customer shall defend, indemnify and hold harmless the Company and its officers, directors, employees, agents, servants and<br />

assigns from and against any and all losses, claims, demands and/or liability for damage to property, injury or de<strong>at</strong>h of any person, or any other liability<br />

incurred by the Company, including all expenses, legal or otherwise, arising out of or rel<strong>at</strong>ed to the equipment or install<strong>at</strong>ion, except to the extent<br />

<strong>at</strong>tributable to the negligence of Company. In no event shall Company’s liability to Customer exceed the reb<strong>at</strong>e amounts.<br />

9. Limited Scope Review - The scope of review by the Company and the Vendor and their Inspector of the install<strong>at</strong>ion of the equipment is solely to<br />

determine whether reb<strong>at</strong>es are payable. It does not include any kind of safety or code review and should not be relied upon as one.<br />

10. Reb<strong>at</strong>e Amounts - The Company will provide reb<strong>at</strong>es for new, qualifying he<strong>at</strong>ing, w<strong>at</strong>er he<strong>at</strong>ing, thermost<strong>at</strong> or boiler reset control equipment, up to<br />

reb<strong>at</strong>e amount indic<strong>at</strong>ed in the program liter<strong>at</strong>ure and within this applic<strong>at</strong>ion. Reb<strong>at</strong>es cannot exceed the purchase price of the equipment.<br />

11. Payment - The Company, through the Vendor, expects to make reb<strong>at</strong>e payments to eligible customers within 6-8 weeks of a s<strong>at</strong>isfactory work<br />

verific<strong>at</strong>ion. The customer must refund any reb<strong>at</strong>e made to the extent the contractor or equipment does not s<strong>at</strong>isfy program requirements.<br />

12. No Warranties - The Company and the Vendor do not endorse, guarantee or warrant any particular contractor, manufacturer or product install<strong>at</strong>ion.<br />

The Company and the Vendor do not make any represent<strong>at</strong>ion of any kind regarding the results to be achieved by the equipment or the adequacy or<br />

safety of such equipment.<br />

13. Changes in the Program - Equipment Incentive Program/Tax Liability Conditions and details of the program are subject to change without prior notice<br />

and reb<strong>at</strong>e offers may increase or decrease over time. The Company reserves the right to modify or termin<strong>at</strong>e the program without notice. Participants<br />

of the program may be subject to tax liability for the value of goods and services received through the program pursuant to st<strong>at</strong>e or federal income tax<br />

codes. The Company is not responsible for any tax liability which may be imposed as a result of receipt of the reb<strong>at</strong>e by the Customer.<br />

14. Contractor Insurance - The Company is not responsible for any damage th<strong>at</strong> may be caused by or arise out of an install<strong>at</strong>ion of any equipment. The<br />

customer is responsible for selecting contractors who are qualified and carry adequ<strong>at</strong>e insurance coverage.<br />

15. Eligible Measures - Only measures included on this applic<strong>at</strong>ion are eligible for reb<strong>at</strong>es. The company will not provide reb<strong>at</strong>e payments for non-eligible<br />

equipment, substitutions or used equipment. The Company has the right to reject any reb<strong>at</strong>e applic<strong>at</strong>ion with ineligible equipment not indic<strong>at</strong>ed on this <strong>form</strong>.<br />

16. Payments Assignable to a Third Party - (a) The Customer may request th<strong>at</strong> the incentive be paid directly to a third party by so indic<strong>at</strong>ing in the<br />

Program applic<strong>at</strong>ion. Notific<strong>at</strong>ion of third-party payment will be sent to the Customer upon submission of the Program applic<strong>at</strong>ion for the purpose<br />

of Customer confirm<strong>at</strong>ion. (b) If no payment choice is made, the Company will send the incentive payment directly to the Customer <strong>at</strong> the address<br />

indic<strong>at</strong>ed in the Program applic<strong>at</strong>ion.<br />
