FEE CONCESSION/FINANCIAL ASSISTANCE FORM (2012-2013)
The process of Fee Concession is based on assessment of need and merit. Selection will be
decided on the basis of information provided in the form and investigations by the Committee.
Candidates may be required to appear for interview. More over concession in tuition fee will be
applicable for two semesters. Each year student will have to apply for the concession on the basis of
their semester results. Amount will be refunded through cheque from Treasurer Office.
PROVIDING FALSE INFORMATION MAY RESULT OF THE FOLLOWING
• Disqualification for award of any future loan/scholarship/Financial Aid
• Refund of all the payment received and or a penalty equal to total scholarship amount.
INSTRUCTIONS FOR FILLING OUT THE SCHOLARSHIP APPLICATION FORM:
Complete the photocopy form and make sure everything is correct and final
Copy all information from photocopied form to the original form
Submit duly completed & verified documents with application form.
For family financial reporting, parents/guardian may be consulted for guidance
Whenever in doubt or lost, seek help from the (Scholarship Officer)
Keep a photocopy of the filled-in original application form for your record
Note:
REQUIRED DOCUMENTS
1. Copy of All academic certificates (Matriculation. Intermediate and BA/BSC/B.COM)
2. B-Form ( بم ) and NIC
3. Copies of last & latest fee receipts of self and siblings
4. Copies of all Bills/ expenditure related documents (if applicable)
5. Income Certificate/Salary/Pay Slip of father/Mother/Guardian dully verified by following
• Head of Institute in case of Govt/Semi Govt
• ILAQA Tehsildar in case of Agriculture
• Union Council in case of Labor
6. Death Certificate of father must be attached. (Orphan Students)
7. Disability Certificate (Disable student)
8. Attach copies of all Utility Bills(Electricity, Gas, PTCL, Water) of previous month
Students of (3 rd ,5 th ,7 th ) semester who are already availing concession in their
tuition fees are instructed to submit only their updated academic performance
Report of previous semester (Spring/Summer 2012).
Students having GPA/CGPA below 2.5 are strictly disallowed to apply for fee
Concession or application form will be rejected by the scholarship office.
Application form along with all required documents must be duly verified by their Head of
Department and submit directly to the office of Scholarship at Students Services center
(SSC).Incomplete applications and after due date will not be entertained.
