In adults with head and neck dermatitis a positive ImmunoCAP assay to Malassezia species<br />
may be useful as such patients may benefit from treatment with oral anti-yeast treatment. 1,2<br />
Patch tests involve formal patch testing to diagnose superimposed allergic c<strong>on</strong>tact<br />
dermatitis, e.g. in adult patients with chr<strong>on</strong>ic hand dermatitis. The aetiology <strong>of</strong> <strong>the</strong> c<strong>on</strong>tinuing<br />
dermatitis is elucidated. 1 Formal patch tests are also useful for deteriorating dermatitis that is<br />
being intensively treated. The patient may have an allergic c<strong>on</strong>tact dermatitis to <strong>on</strong>e <strong>of</strong> <strong>the</strong><br />
comp<strong>on</strong>ents <strong>of</strong> <strong>the</strong> current topical treatment. 1<br />
The Atopy Patch Test (APT) is used to diagnose type IV hypersensitivity reacti<strong>on</strong>s and is<br />
used to determine food and aeroallergen triggers for dermatitis. It is a specialised, timec<strong>on</strong>suming<br />
procedure that is highly specific. but has lower levels <strong>of</strong> sensitivity. 6 It is not <strong>of</strong>ten<br />
used in adults with atopic dermatitis. 1,2,5,6<br />
If immunodeficiency is suspected with atopic dermatitis, tests like HTLV-1, HIV,<br />
immunoglobulin subtypes, T and B cell numbers and functi<strong>on</strong>s will need to be d<strong>on</strong>e. If<br />
sec<strong>on</strong>dary bacterial infecti<strong>on</strong>s are suspected, pus swabs will be necessary. For suspected<br />
herpes infecti<strong>on</strong> a Tzanck smear, an immun<strong>of</strong>luorescence slide test or electr<strong>on</strong><br />
microscopy will give a quick c<strong>on</strong>firmati<strong>on</strong>. 1<br />

Preventi<strong>on</strong> <strong>of</strong> atopic dermatitis<br />
Preventi<strong>on</strong> <strong>of</strong> atopic dermatitis, next to a cure for <strong>the</strong> c<strong>on</strong>diti<strong>on</strong>, would be first prize for<br />
practiti<strong>on</strong>ers treating and researching this c<strong>on</strong>diti<strong>on</strong>. Many exciting strategies for preventi<strong>on</strong><br />
are being investigated, but need fur<strong>the</strong>r research. Our existing advice has not met with much<br />
progress in <strong>the</strong> last decade <strong>of</strong> investigati<strong>on</strong>.<br />
Primary preventi<strong>on</strong><br />
Attempts to provide allergen-free diets to pregnant mo<strong>the</strong>rs, so popular in <strong>the</strong> early 1990s,<br />
have failed to prevent <strong>the</strong> development <strong>of</strong> allergy or atopic dermatitis. Food allergen<br />
avoidance during pregnancy and early life is unhelpful; in fact, it may even promote<br />
atopy. 1 Allergen avoidance in breastfeeding mo<strong>the</strong>rs (avoiding potentially allergenic foods),<br />
