<strong>Certification</strong> <strong>in</strong> <strong>Genetic</strong> Counsell<strong>in</strong>g<br />

A. DEFINITION AND DUTIES<br />

<strong>Genetic</strong> counsellors have background tra<strong>in</strong><strong>in</strong>g <strong>in</strong> a wide variety of discipl<strong>in</strong>es<br />

<strong>in</strong>clud<strong>in</strong>g nurs<strong>in</strong>g, science, social work, education and psychology.<br />

The HGSA certified genetic counsellor is part of the team deliver<strong>in</strong>g genetic<br />

services. The role of the genetic counsellor is varied and depends on the<br />

needs of the particular genetic cl<strong>in</strong>ic or their employment. The HGSA certified<br />

genetic counsellor who works <strong>in</strong> a cl<strong>in</strong>ical sett<strong>in</strong>g has a primary role <strong>in</strong> the<br />

provision of genetic counsell<strong>in</strong>g.<br />

In addition to the provision of genetic counsell<strong>in</strong>g the core cl<strong>in</strong>ic duties may<br />

<strong>in</strong>clude:<br />

• genetic cl<strong>in</strong>ic organisation<br />

• <strong>in</strong>take sessions before the diagnostic visit<br />

• follow-up of families and <strong>in</strong>dividuals<br />

• responsibility for documentation that <strong>in</strong>cludes pedigree tak<strong>in</strong>g and<br />

record<strong>in</strong>g of social, economic and psychological factors.<br />

<strong>Genetic</strong> counsellors may also be <strong>in</strong>volved with screen<strong>in</strong>g programmes, DNA<br />

test<strong>in</strong>g programmes, research, genetic education and promotional activities<br />

<strong>in</strong>clud<strong>in</strong>g assist<strong>in</strong>g support groups. Some counsellors may have a major<br />

<strong>in</strong>volvement <strong>in</strong> a specific area, eg. genetic education.<br />

B. TRAINING AND CERTIFICATION<br />

The HGSA set up a Board of Censors for <strong>Genetic</strong> Counsell<strong>in</strong>g (hereafter<br />

called the Board), to oversee the tra<strong>in</strong><strong>in</strong>g of genetic counsellors <strong>in</strong> 1989. The<br />

Board consists of a certified cl<strong>in</strong>ical geneticist, a specialist practitioner <strong>in</strong><br />

counsell<strong>in</strong>g, eg a genetic social worker, and a further three to five genetic<br />

counsellors. A majority of Board members are HGSA certified.<br />

The Board recommends that any person <strong>in</strong>volved <strong>in</strong> genetic counsell<strong>in</strong>g<br />

should be tra<strong>in</strong>ed <strong>in</strong> genetics, pr<strong>in</strong>ciples and practice of counsell<strong>in</strong>g, and<br />

<strong>in</strong>terview techniques. The Board has produced certification <strong>in</strong> genetic<br />

counsell<strong>in</strong>g guidel<strong>in</strong>es that should be followed by all candidates.<br />

The process to obta<strong>in</strong> general genetic counsell<strong>in</strong>g certification is divided <strong>in</strong>to<br />

Part I and Part II. There are fees for both Part I and Part II. Part I certification<br />

<strong>in</strong>volves undertak<strong>in</strong>g the necessary course work <strong>in</strong> genetics and counsell<strong>in</strong>g<br />

to complete a checklist of requirements. There are also graduate diploma of<br />

genetic counsell<strong>in</strong>g and masters of genetic counsell<strong>in</strong>g courses which are<br />

recognised by the Board as fulfill<strong>in</strong>g Part I requirements. Part II certification<br />

<strong>in</strong>volves a m<strong>in</strong>imum of two years full time equivalent work as a genetic<br />

counsellor, dur<strong>in</strong>g which time candidates must keep a log book of 100 cases,<br />

and complete 20 written case studies. Candidates must undertake both<br />

genetic and counsell<strong>in</strong>g supervision for the duration of their Part II certification<br />

time period.<br />

Candidates are encouraged to make regular submissions to the Board, which<br />

accepts candidate submissions twice annually. The Board undertakes to give<br />

guidance about tra<strong>in</strong><strong>in</strong>g requirements to <strong>in</strong>dividual candidates, and provides<br />

feedback on all candidates written submissions. The Board has the discretion<br />

to ask <strong>in</strong>dividual candidates to complete additional requirements to achieve<br />

certification.<br />

Speciality tra<strong>in</strong><strong>in</strong>g <strong>in</strong> the area of cancer genetic counsell<strong>in</strong>g is available; the<br />

certification process is divided <strong>in</strong>to Part I and Part II. Part I tra<strong>in</strong><strong>in</strong>g is as<br />

described above. Part II tra<strong>in</strong><strong>in</strong>g is ga<strong>in</strong>ed through employment equivalent to<br />

one full time year, <strong>in</strong> a recognised cl<strong>in</strong>ic or unit. In addition, 10 written case<br />

studies and a logbook of 50 cases must be submitted. Counsell<strong>in</strong>g and<br />

genetic supervision is as required for general genetic counsell<strong>in</strong>g tra<strong>in</strong><strong>in</strong>g. If a<br />

person certified <strong>in</strong> cancer genetic counsell<strong>in</strong>g wishes to then tra<strong>in</strong> <strong>in</strong> general<br />

genetic counsell<strong>in</strong>g, they would need to complete a further one year<br />

equivalent of supervised full time work, and submit a further 15 case studies<br />

and a logbook of 50 cases.<br />

Copies of the Guidel<strong>in</strong>es for <strong>Certification</strong> <strong>in</strong> <strong>Genetic</strong> Counsell<strong>in</strong>g produced by<br />

the Board can be obta<strong>in</strong>ed from the HGSA Secretariat.<br />

Candidates who have tra<strong>in</strong>ed as genetic counsellors outside Australia may<br />

apply to the Board to determ<strong>in</strong>e their eligibility for HGSA certification as a<br />

genetic counsellor.<br />
