Academic Boosters of Crestview—This was the 27th annual presentation of the Academic Boosters of Crestview’s Appreciation evening. In honor of this being the 50th Anniversary of Crestview Schools, we wanted to honor all our past and present honorees, as well as, the following for their time and efforts in guiding our students and helping to make the SAIL night a huge success: Bill Seder, Larry Messmore, Scott Will, Dan Stevens, Judi Roman, Rhonda Carey, Michelle Vipperman—for helping locate past honorees addresses, Deb Bernhard, Lissa Philpotts, Denise Perfetta, Autumn Harmon, Chandler Harmon, Cory Perfetta, Jordyn Gray, Alexis Cullion, Brette Purvis, Gretchen Sauder, Joe Frazier, Katlyn Hulit, Courtny Albion from Ashland Times Gazette, Tara’s Florist, What’s for Dessert, Meijer’s, Sam’s Club and to all the parents for the encouragement and support of their sons and daughters! Your efforts afforded Crestview’s Track athletes the opportunity to proudly showcase their talents to the community. V O L U M E 2 0 1 1 - 1 2 , I S S U E 6 Community & General Information CRESTVIEW SPORTS PHYSICALS FOR 2012-13 SCHOOL YEAR AND ATHLETIC NEWS Please Note: Physicals will be performed at Crestview High School on Wednesday, July 18, 2012. Physicals for the middle school students will begin at 6:00 PM and physicals for the high school students will begin at 6:30 PM You must complete the OHSSA Pre-Participation Physical Exam Forms packet (located in the high school or middle school offices or on the Crestview’s website under the Athletics tab. Make sure it is filled out completely and signed by your parent/guardian. Cost is $10.00. Upcoming Important Athletic Dates: July 18, 2012 Physical Night at Crestview High School 6:00 PM for MS and 6:30 PM for HS students. $10 fee. (Checks made out to Crestview High School) July 30, 2012 Official practice begins for high school football and golf. July 31, 2012 7:00 PM—Parent Information Night. Mandatory meeting for HS & MS Fall sports athletes and their parents. HS Aud. August 6, 2012 Official practice begins for all other HS/MS fall sports Athletic Forms & Athletic Fee: All athletic forms (Emergency Medical Form, Athletic Handbook Acknowledge Form, and Medical Insurance Coverage Form) need to be turned in to your coach the first week of practice. These forms will be available during the Physical Night and during the Parent Information Night. The $35.00 Athletic Fee must be paid to the athletic director or athletic secretary by the first scheduled sporting event. Crestview Educational Foundation… CEF The Crestview Educational Foundation is celebrating their 3rd year anniversary by awarding five $500 scholarships to members of the Class of 2012 and three teacher grants of $250. If you are interested in contributing, you may send any amount to The Crestview Educational Foundation, P.O. Box 1701, Mansfield, OH 44901. Crestview School Breakfast Programs Crestview will continue to offer a breakfast program at both the Middle and Elementary Schools free to all students for the 2012- 2013 school year. An application for a waiver to opt out of the breakfast program at the High School is planned due to financial considerations and time constraints. P A G E 7 CAFETERIA NEWS Lunch Price Increase Due to requirements in the Healthy, Hunger- Free Kids Act of 2010, we will be raising the price of an Elementary lunch from $2.30 to $2.35 and the Middle & High Schools from $2.50 to $2.55 for the 2012-2013 school year. EZpay – Last year, many parents took advantage of paying for school fees and lunches by using EZpay, our on-line payment system. All you need is your student’s 6-digit ID number, click on the EZpay link on the C r e s t v i e w w e b s i t e a t (www.crestviewschools.net) and you can set up an account. Even if you don’t pay for lunches or school fees using EZpay, you can still set up an EZpay account to view your student’s lunch account balance or view their lunch history. You can obtain your student’s 6-digit ID number by contacting the Food Service department at 419-895-1700, ext. 5002 or by calling the secretary in your student’s building. NOTE: There is a convenience charge of $2.00 each time you use the EZpay system to make a payment. Charging Lunches – Our guidelines for lunch charges will remain the same. Only a complete lunch may be charged – no ala carte items may be charged. Students in the Elementary are allowed three charges; Middle School two charges; and High School one charge. When a charge is paid, a student is then eligible to charge again. Free & Reduced Meals – Free and Reduced applications will be sent home with your children on the first day of school. They should be available on the school web site in early August. If you are direct certified, you will receive a letter from the Food Service Department during August and you will not need to fill out an application even though one will still come home with your student(s).
