DEFINITIONS OF TERMS<br />

Date <strong>of</strong> Activity- Actual date activity took place. Includes preparation at <strong>of</strong>fice<br />

and on jobsite activities.<br />

Activity – Direct participation by applicator in identifying infestations,<br />

recommendations, estimates, bids, and disclosures, making contracts, submitting<br />

bids or performing services, preparing structures for fumigation, applying<br />

fumigants, and clearing structures. Applicators must actively participate in 9 <strong>of</strong><br />

the following tasks on each <strong>of</strong> three fumigations to receive credit. If an<br />

applicator participates in additional fumigations within the license year,<br />

there is no requirement to record the additional fumigations on this form.<br />

1. Inspect structure and identify <strong>pest</strong>(s) prior to fumigation<br />

2. Prepare disclosure documents pursuant to the provisions <strong>of</strong> §599.4 <strong>of</strong> the<br />

SPCS regulations<br />

3. Measure structure and calculate volume<br />

4. Calculate dosage <strong>of</strong> Fumigant<br />

5. Notify local fire or police authority as appropriate<br />

6. Secure materials left inside structure, check pilot lights & appliances<br />

7. Tarp, place snakes, or otherwise seal structure for fumigation<br />

8. Post the structure and secure entrances to the structure<br />

9. Instruct the person (guard) on duty at the site on responsibilities and safety<br />

precautions<br />

10. Provide onsite supervision during application (only one applicator per<br />

company may claim provision <strong>of</strong> onsite supervision)<br />

11. Set up equipment including splash pan and fans for<br />

12. Introduce fumigant and warning agent (if required) to the structure<br />

13. Aerate structure<br />

14. Take down tarps, remove snakes, remove locks, or otherwise remove<br />

sealing material<br />

15. Clear the structure<br />

16. Provide onsite supervision during aeration and clearing(only one<br />

applicator per company may claim provision <strong>of</strong> onsite supervision)<br />

17. Store and/or dispose <strong>of</strong> fumigant containers<br />

18. Prepare the report <strong>of</strong> fumigation required by §599.9(j) <strong>of</strong> the SPCS<br />

regulations<br />
