The Board reviewed and considered the licensure information for Carolyn C. Jackson,
R.D.H., which was prepared by Ms. Smith.
Motion by Dr. Kyger, second by Ms. Naber, to approve Ms. Jackson’s
reinstatement application for dental hygiene licensure in the state of Ohio.
Motion carried unanimously.
NOEL P. PATTON, D.D.S.
The Board reviewed and considered the licensure information for Noel P. Patton,
D.D.S., which was prepared by Ms. Smith.
Motion by Dr. Kaye, second by Dr. Leffler, to approve Dr. Patton’s
reinstatement application for dental licensure in the state of Ohio.
Motion carried unanimously.
KIMBERLY A. RICHARDS, R.D.H.
The Board reviewed and considered the licensure information for Kimberly A.
Richards, R.D.H., which was prepared by Ms. Smith.
Motion by Ms. Staley, second by Dr. Kyger, to approve Ms. Richards’
reinstatement application for dental hygiene licensure in the state of Ohio.
Motion carried unanimously.
ANESTHESIA COMMITTEE REPORT
REVIEW OF PROVISIONAL CONSCIOUS SEDATION
PRIVILEGE(S)
Dr. Wallace stated that the following individuals have applied for Conscious Sedation
permits. He explained that the Anesthesia Committee has reviewed the applications and
the applicants are recommended to receive provisional privileges:
Dr. Kapil Davar – Sylvania, Ohio
- Oral for children 12 years or younger
Dr. Ronald D. Fowler – Columbus, Ohio
