<strong>the</strong> <strong>HSE</strong> South Region<br />

As an interim measure Waterford <strong>Child</strong>ren <strong>and</strong> Family<br />

<strong>Service</strong>s will continue to engage with <strong>the</strong> RAISE governance<br />

group to improve <strong>the</strong> robustness <strong>and</strong> reliability <strong>of</strong> <strong>the</strong> child<br />

protection information recorded.<br />

The management team will liaise with <strong>the</strong> National Lead for<br />

NCCIS to ensure <strong>the</strong> development <strong>of</strong> a robust National<br />

Information system which reflects <strong>the</strong> current service delivery<br />

model.<br />

Waterford <strong>Child</strong>ren <strong>and</strong> Family <strong>Service</strong>s have developed a<br />

secure Excel database system to capture relevant information<br />

pertaining to welfare cases. In conjunction with this a<br />

recording system for referrals from <strong>the</strong> child protection<br />

system to <strong>the</strong> LAP has also been developed.<br />

A similar database <strong>and</strong> recording system has been created<br />

for <strong>the</strong> local Community Adolescent Team.<br />

Theme 6: Use <strong>of</strong> Information<br />

The LHA was not compliant with <strong>the</strong> st<strong>and</strong>ard in <strong>the</strong> following respects:<br />

The LHA did not have robust record keeping <strong>and</strong> file management systems in place<br />

to support <strong>the</strong> management <strong>of</strong> child protection <strong>and</strong> welfare information, in particular<br />

in relation to <strong>the</strong> filing, archiving <strong>and</strong> secure storage <strong>of</strong> records sent to <strong>the</strong> <strong>of</strong>fsite<br />

archiving facility.<br />

The LHA did not undertake regular audits to evaluate <strong>the</strong> record-keeping <strong>and</strong> file<br />

management system <strong>and</strong> practices.<br />

19. Action required:<br />

The LHA should put in place a robust system for <strong>the</strong> secure storage <strong>and</strong> archiving <strong>of</strong><br />

all confidential information.<br />
