period.
2 Implement drug-drug, drug-allergy checks. Enable and have access to at least one internal or external formulary. No exclusion.

6 Maintain an active allergy list.
Maintain an active allergy list for at least 80% of unique patient visits
and at least one entry.
No exclusion.
7 Record Demographics.
8 Record and chart changes in vital signs.
More than 50% of all unique patients have demographics recorded
as structured data
Record vital signs and children growth charts of more than 50% of
unique patient visits. Required age for blood pressure is three years
or older.
No exclusion.
1. Any EP who sees no patients 3 years or older is excluded from recording
blood pressure.
2. Any EP who believes that height, weight and blood pressure have no
relevance to their scope of practice are excluded from recording them.
3. Any EP who believes that height and weight are relevant to their
practice, but blood pressure is not, is excluded from recording blood
pressure.
4. Any EP who believes that blood pressure is relevant to their practice,
but height and weight are not, is excluded from recording height and
weight.
9 Record smoking status.
More than 50% of all unique patients 13 years and older have
smoking status recorded as structured data.
Any EP who sees no patients 13 years or older.
10 Launch/track clinical decision support rule. Implement at least one clinical decision support rule. No exclusion.

Provide patients the ability to view online,
download and transmit their health
information.
Provide 50% or more of all unique patients seen during the EHR
reporting period, online access to their health inforamtion within 4
business days.
Any EP that has no requests from patients or their agents for an electronic
copy of patient health information during the EHR reporting period.
12 Provide clinical summaries.
13 Protect electronic health information.
Provide clinical summaries (EPs) and discharge summary (hospitals)
for at least 50% of all patients.
Conduct or review a security risk analysis in accordance with the
requirements under 45 CFR 164.308(a)(1) and implement security
updates as necessary and correct identified security deficiencies as
part of the provider’s risk management process.
Any EP who has no office visits during the EHR reporting period.
No exclusion.

Menu Set Measures (Choose Five of Nine)
1 Drug formulary checks.
Successful ongoing submission of electronic syndromic surveillance
data from Certified EHR Technology to a public health agency for the
entire EHR reporting period.
Any EP who meets one or more of the following criteria may be excluded:
1. The EP is not in a category of providers that collects ambulatory
syndromic surveillance information on their patients during the EHR
reporting period.
2. The EP operates in a jurisdication for which no public health agency
is capable of recieveing electronic syndromic surveillance data in the
specific standards required by the CERHT at the start of their EHR
reporting period.
3. The EP who operates in a jurisdiction where no public health agency
provides information timely on capability to recieve syndromic
surveillance data.
4. The EP operates in a jursidiction for which no public health agency that
is capable of accepting the specific standards required by CERHT at the
start of their EHR reporting period can enroll additonal EPs.
2 Import and store clinical lab test results.
3 Patient lists by condition.

EP during the reporting period are available through Certified EHR
Technology.
More than 20% of all unique patients seen by the EP during the
reporting period have a structured data entry for one or more firstdegree
relatives.
Successful ongoing submission of cancer case information from
Certified EHR Technology to a cancer registry for the entire EHR
reporting period.
Successful ongoing submission of specific case information from
Certified EHR Technology to a specialized registry for the entire EHR
reporting period.
Submit at least one test/follow-up of electronic data to
immunization registries or immunization information systems.
Submit at least one test/follow-up of syndromic surveillance data to
public agencies.
Send reminder to 20% or more patients per patient preference for
preventative follow up care. Patients must be 65 years or older or 5
years old or younger.
Any EP who has no office visits during the EHR reporting period.
Previously EP was required to perform one test. This measure now requires
ongoing submissions of data to a public health agency.
Any EP who has no office visits during the EHR reporting period.
Any EP that meets one or more of the following criteria may be excluded:
1. The EP doesn’t diagnose or directly treat cancer.
2. The EP operates in a jursidiction for which no public health agency is
capable of receiving electronic cancer case information in the specific
standards required for the CERHT at the beginning of their EHR
reporting period.
3. The EP operates in a jurisdiction where no PHA provides information
timely on capability to recive electronic cancer case information.
4. The EP operates in a jurisdiction for which no public health agency that
is capable of recieving electronic cancer case information in the specific
standards required for CERHT at the beginning of their EHR reporting
perios can enroll additional EPs.
Any EP that meets one or more of the following criteria may be excluded:
1. The EP doesn’t diagnose or directly treat any disease associated with a
specialized registry sponsored by a national specialty society for which
the EP is eligible, or the public health agenices in their jurisdiction.
2. The EP operates in a jurisdiction for which no specialized registry
sponsored by a public health agency or by a national speciality socitey
for which the EP is eligble is capable of reciving electronic specific
case information in the specific standards required by CERHT at the
beginning of their EHR reporting period.
3. The EP operates ina jurisdiction where no public health agency
or national specialty society for which the EP is eligble provides
information timely on capability to recieve information into their
specialized registries.
4. The EP operates ina jurisdiction for which no specialized registry for
which the EP is elgible that is capable of recieveing electronic specific
case information in the specific standards required by CERHT at the
beginning of their EHR reporting period can enroll additional EPs.
Any EP who administers no immunizations during the EHR reporting
period, where no immunization registry has the capacity to receive the
information electronically, or where it is prohibited.
Any EP who does not collect any reportable syndromic information on
their patients during the EHR reporting period, does not submit such
information to any public health agency that has the capacity to receive the
information electronically, or if it is prohibited.
Any EP who has no patients 65 years or older or 5 years or younger with
records maintained using certified EHR technology.
