Record vital signs and children growth charts of more than 50% of<br />

unique patient visits. Required age for blood pressure is three years<br />

1. Any EP who sees no patients 3 years or older is excluded from recording<br />

blood pressure.<br />

2. Any EP who believes that height, weight and blood pressure have no<br />

relevance to their scope of practice are excluded from recording them.<br />

3. Any EP who believes that height and weight are relevant to their<br />

practice, but blood pressure is not, is excluded from recording blood<br />

pressure.<br />

4. Any EP who believes that blood pressure is relevant to their practice,<br />

but height and weight are not, is excluded from recording height and<br />

weight.<br />

9 Record smoking status.<br />

More than 50% of all unique patients 13 years and older have<br />

smoking status recorded as structured data.<br />

Any EP who sees no patients 13 years or older.<br />

10 Launch/track clinical decision support rule. Implement at least one clinical decision support rule. No exclusion.<br />

Provide patients the ability to view online,<br />

updates as necessary and correct identified security deficiencies as<br />

part of the provider’s risk management process.<br />

1 Drug formulary checks.<br />

Successful ongoing submission of electronic syndromic surveillance<br />

data from Certified EHR Technology to a public health agency for the<br />

entire EHR reporting period.<br />

Any EP who meets one or more of the following criteria may be excluded:<br />

1. The EP is not in a category of providers that collects ambulatory<br />

syndromic surveillance information on their patients during the EHR<br />

reporting period.<br />

2. The EP operates in a jurisdication for which no public health agency<br />

is capable of recieveing electronic syndromic surveillance data in the<br />

specific standards required by the CERHT at the start of their EHR<br />

reporting period.<br />

3. The EP who operates in a jurisdiction where no public health agency<br />

provides information timely on capability to recieve syndromic<br />

surveillance data.<br />

4. The EP operates in a jursidiction for which no public health agency that<br />

is capable of accepting the specific standards required by CERHT at the<br />

start of their EHR reporting period can enroll additonal EPs.<br />

2 Import and store clinical lab test results.<br />

reporting period have a structured data entry for one or more firstdegree<br />

relatives.<br />

Successful ongoing submission of cancer case information from<br />

Certified EHR Technology to a cancer registry for the entire EHR<br />

reporting period.<br />

Successful ongoing submission of specific case information from<br />

Certified EHR Technology to a specialized registry for the entire EHR<br />

reporting period.<br />

Submit at least one test/follow-up of electronic data to<br />

immunization registries or immunization information systems.<br />

Submit at least one test/follow-up of syndromic surveillance data to<br />

public agencies.<br />

Send reminder to 20% or more patients per patient preference for<br />

preventative follow up care. Patients must be 65 years or older or 5<br />

years old or younger.<br />

Any EP who has no office visits during the EHR reporting period.<br />

Previously EP was required to perform one test. This measure now requires<br />

ongoing submissions of data to a public health agency.<br />

Any EP who has no office visits during the EHR reporting period.<br />

Any EP that meets one or more of the following criteria may be excluded:<br />

1. The EP doesn’t diagnose or directly treat cancer.<br />

2. The EP operates in a jursidiction for which no public health agency is<br />

capable of receiving electronic cancer case information in the specific<br />

standards required for the CERHT at the beginning of their EHR<br />

reporting period.<br />

3. The EP operates in a jurisdiction where no PHA provides information<br />

timely on capability to recive electronic cancer case information.<br />

4. The EP operates in a jurisdiction for which no public health agency that<br />

is capable of recieving electronic cancer case information in the specific<br />

standards required for CERHT at the beginning of their EHR reporting<br />

perios can enroll additional EPs.<br />

Any EP that meets one or more of the following criteria may be excluded:<br />

1. The EP doesn’t diagnose or directly treat any disease associated with a<br />

specialized registry sponsored by a national specialty society for which<br />

the EP is eligible, or the public health agenices in their jurisdiction.<br />

2. The EP operates in a jurisdiction for which no specialized registry<br />

sponsored by a public health agency or by a national speciality socitey<br />

for which the EP is eligble is capable of reciving electronic specific<br />

case information in the specific standards required by CERHT at the<br />

beginning of their EHR reporting period.<br />

3. The EP operates ina jurisdiction where no public health agency<br />

or national specialty society for which the EP is eligble provides<br />

information timely on capability to recieve information into their<br />

specialized registries.<br />

4. The EP operates ina jurisdiction for which no specialized registry for<br />

which the EP is elgible that is capable of recieveing electronic specific<br />

case information in the specific standards required by CERHT at the<br />

beginning of their EHR reporting period can enroll additional EPs.<br />

Any EP who administers no immunizations during the EHR reporting<br />

period, where no immunization registry has the capacity to receive the<br />

information electronically, or where it is prohibited.<br />

Any EP who does not collect any reportable syndromic information on<br />

their patients during the EHR reporting period, does not submit such<br />

information to any public health agency that has the capacity to receive the<br />

information electronically, or if it is prohibited.<br />

Any EP who has no patients 65 years or older or 5 years or younger with<br />

records maintained using certified EHR technology.<br />
