The hazards noted are in violation of the Florida Fire Prevention Code and/or Miami-Dade County Code. Nothing in this <strong>report</strong> supersedes any<br />

previously written, still existing violations for this occupancy. You are ordered to comply with the corrective measures within the time<br />

Please Note: A life safety permit required by County Code# 14-53 to operate within Miami-Dade County will be issued upon correction of all outstanding<br />

violations and payment of fees assessed. Failure to obtain the required Life Safety Permit will result in Enforcement Actions including fines,<br />

In accordance with the Code of Metropolitan Dade County part III, chapter 8CC-5; A violator who has been served with a civil violation notice may request<br />

an administrative hearing before a Hearing Officer to appeal the decision of the Code Inspector which resulted in the issuance of the civil violation notice.<br />

The request for appeal must be submitted in writing no later than twenty (20) calendar days after the service of the notice. Failure of the named violator<br />

to appeal the decision of the Code Inspector within the prescribed time period shall constitute a waiver of the violator's right to administrative hearing<br />

before the Hearing Officer. A waiver of the right to administrative hearing shall be treated as an admission of the violation and penalties may be assessed<br />

Violation: Code of Miami-Dade County 14-53(F) All applications for permits pursuant to this<br />

section shall be on a form prescribed by the Fire Department having jurisdiction and accompanied<br />

by the required fee. The fees shall be established by administrative order of the County Manager<br />

and approved by the Board of County Commissioners for permits within the jurisdiction of the<br />

Metropolitan Dade County Fire Department. The fees shall be established by the governing body of<br />

the City of Miami Beach, the City of Miami, the City of Hialeah and the City of <strong>Coral</strong> Gables for such<br />

permits within the respective jurisdictions of the aforesaid municipalities.<br />

runner service available to respond to the protected premises at any and all times within two (2)<br />

hours of notification of the runner service of any alarm or supervisory signal, and within four (4)<br />

hours of notification of any trouble signal. Existing <strong>fire</strong> alarm systems shall comply with Section<br />

14-66 (4) of this Code not later than five hundred forty-five (545) days after the effective date of this<br />

ordinance. The authority having jurisdiction shall have the authority to extend the time for<br />

compliance with Section 14-66 (4) of this Code for existing <strong>fire</strong> alarm systems to not later than one<br />

thousand ninety-five (1,095) days after the effective date of this ordinance if the authority having<br />

jurisdiction determines that compliance within five hundred forty-five (545) days is not reasonable<br />

under all the facts and circumstances.<br />

Correction: Enter into a contract with a <strong>fire</strong> alarm contractor for both the service of the <strong>fire</strong> alarm<br />

system and for runner service for any alarm, supervisory signal, or trouble signal. Service must<br />

include all tests, <strong>inspection</strong>s, and repairs required by NFPA 72. Runner must be at the premises and<br />

initiate corrective action within 2 hours of an alarm or supervisory signal and within 4 hours of a<br />

trouble signal. Provide a copy of the contract for review by the <strong>fire</strong> inspector.<br />

Violation: NFPA 101 7.1.3.2.1 Where this Code requires an exit to be separated from other<br />

parts of the building, the separating construction shall meet the requirements of Section 8.2 and the<br />

following: 1) The separation shall have a minimum 1-hour <strong>fire</strong> resistance rating where the exit<br />

connects three or fewer stories. 2) The separation shall have a minimum 2-hour <strong>fire</strong> resistance<br />

rating where the exit connects four or more stories, unless one of the following conditions exists: a)<br />

resistance rating, unless otherwise permitted by 7.1.3.2.1(6).<br />
