Lastly, mounting global uncertainty underlines <strong>the</strong> need for strategies to manage <strong>the</strong> risk inherent<br />
in <strong>the</strong> traditi<strong>on</strong>al approach to food security, in order to mitigate <strong>the</strong> impacts of unforeseen shocks and<br />
a<strong>da</strong>pt to more permanent changes.<br />
Target 1C of <strong>the</strong> Millennium Development Goals —to halve, between 1990 and <str<strong>on</strong>g>20</str<strong>on</strong>g>15, <strong>the</strong><br />
proporti<strong>on</strong> of people who suffer <strong>from</strong> hunger— remains a pending issue in <strong>the</strong> regi<strong>on</strong>. This is<br />
compounded by <strong>the</strong> challenge of reducing obesity and dealing better with <strong>the</strong> ec<strong>on</strong>omic impacts of crises<br />
and natural disasters due to extreme wea<strong>the</strong>r events or natural phenomena (FAO, <str<strong>on</strong>g>20</str<strong>on</strong>g>10a; <str<strong>on</strong>g>20</str<strong>on</strong>g>10b; <str<strong>on</strong>g>20</str<strong>on</strong>g>11).<br />
5. Reproductive rights and sexual and reproductive health<br />
According to <strong>the</strong> Programme of Acti<strong>on</strong> of <strong>the</strong> Internati<strong>on</strong>al C<strong>on</strong>ference <strong>on</strong> Populati<strong>on</strong> and Development<br />
(ICPD), reproductive rights derive <strong>from</strong> <strong>the</strong> basic right of all couples and individuals to decide freely and<br />
resp<strong>on</strong>sibly <strong>the</strong> number, spacing and timing of <strong>the</strong>ir children and to have <strong>the</strong> informati<strong>on</strong> and means to do<br />
so, and <strong>the</strong> right to attain <strong>the</strong> highest stan<strong>da</strong>rd of sexual and reproductive health. It also includes <strong>the</strong>ir right to<br />
make decisi<strong>on</strong>s c<strong>on</strong>cerning reproducti<strong>on</strong> free of discriminati<strong>on</strong>, coerci<strong>on</strong> and violence, as expressed in<br />
human rights documents”. In this framework, am<strong>on</strong>g o<strong>the</strong>r matters <strong>the</strong> Programme of Acti<strong>on</strong> includes<br />
objectives referring to educati<strong>on</strong> (especially for girls) and to greater reducti<strong>on</strong> of infant, child and maternal<br />
mortality, which requires measures to broaden and improve access to sexual and reproductive health care.<br />
These objectives were fur<strong>the</strong>r reinforced with <strong>the</strong> adopti<strong>on</strong> of <strong>the</strong> Millennium Development Goals. In <str<strong>on</strong>g>20</str<strong>on</strong>g>07,<br />
target 5B —to achieve, by <str<strong>on</strong>g>20</str<strong>on</strong>g>15, universal access to reproductive health— was included in Millennium<br />
Development Goal 5.<br />
Latin America and <strong>the</strong> Caribbean is <strong>the</strong> regi<strong>on</strong> which has made <strong>the</strong> most headway in signing<br />
internati<strong>on</strong>al accords <strong>on</strong> human rights, including those which refer to reproductive rights. A number of<br />
countries have adopted nati<strong>on</strong>al legislati<strong>on</strong> expressly recognizing sexual and reproductive rights and <strong>the</strong><br />
great majority have adopted policies and programmes designed to promote <strong>the</strong> realizati<strong>on</strong> of reproductive<br />
rights, broaden access to sexual and reproductive health services and improve <strong>the</strong>ir quality.<br />
Several indicators relating to this field have shown improvements since <strong>the</strong> 1990s and particularly<br />
since <str<strong>on</strong>g>20</str<strong>on</strong>g>04. In <strong>the</strong> latter period, unmet family planning needs fell, <strong>the</strong> coverage of prenatal care and<br />
skilled birth atten<strong>da</strong>nce rose and infant mortality came down. Data <strong>from</strong> <strong>the</strong> Inter-Agency Group for<br />
Child Mortality Estimati<strong>on</strong> show a drop in infant mortality <strong>from</strong> 41 per 1,000 live births to 19 (figures<br />
<strong>from</strong> CEPALSTAT).<br />
Although maternal mortality declined in 1997-<str<strong>on</strong>g>20</str<strong>on</strong>g>05, <strong>the</strong> absolute number of maternal deaths<br />
virtually stood still and many of <strong>the</strong>se appear to have been <strong>from</strong> avoi<strong>da</strong>ble causes.<br />
A particularly important area of c<strong>on</strong>cern for <strong>the</strong> regi<strong>on</strong> is reproductive health during adolescence.<br />
As menti<strong>on</strong>ed previously, early fertility is <strong>on</strong>e of <strong>the</strong> factors involved in <strong>the</strong> intergenerati<strong>on</strong>al<br />
reproducti<strong>on</strong> of poverty, since it obliges households to distribute scarce resources more thinly and makes<br />
it more difficult for mo<strong>the</strong>rs to participate in <strong>the</strong> labour market or c<strong>on</strong>tinue with <strong>the</strong>ir educati<strong>on</strong>. Poor<br />
children develop at a disadvantage in terms of health and access to educati<strong>on</strong> systems. This erodes <strong>the</strong>ir<br />
human capital and c<strong>on</strong>tributes to <strong>the</strong> reproducti<strong>on</strong> of poverty in <strong>the</strong> l<strong>on</strong>g run (ECLAC, <str<strong>on</strong>g>20</str<strong>on</strong>g>11a). Adolescent<br />
fertility is high in Latin America and <strong>the</strong> Caribbean and, unlike in o<strong>the</strong>r regi<strong>on</strong>s, shows no sign of<br />
systematically decreasing. The adult fertility rate has not decreased ei<strong>the</strong>r and most of <strong>the</strong>se pregnancies<br />
are unplanned. The reducti<strong>on</strong> in adolescent fertility since 1990 has been very slight and <strong>the</strong> high rates now<br />
observed in <strong>the</strong> regi<strong>on</strong> are exceeded <strong>on</strong>ly by Sub-Saharan Africa (United Nati<strong>on</strong>s, <str<strong>on</strong>g>20</str<strong>on</strong>g>10b).
