HSE COMMITTEE<br />
h. Committees must maintain meeting minutes from the meeting. <strong>The</strong> meeting minutes must be<br />
communicated or available to all employees.<br />
i. <strong>The</strong> committee must establish a system to allow the members to obtain HSE related suggestions.<br />
j. Management must respond to all HSE committee recommendations before the next meeting,<br />
or within 30 days, whichever happens first.<br />
k. <strong>The</strong> committee must establish procedures for investigating all HSE related incidents including<br />
injury accidents, illnesses, deaths, chemical spills and fires. (This does not mean the committee is<br />
required to conduct the investigations).<br />
l. <strong>The</strong> HSE Committee must assess the HSE Committee process annually and make corrections<br />
and/or improvements as necessary for making the process more efficient and effective.<br />
2. HSE COMMITTEE MEETING AGENDA—<strong>The</strong> following topics must be covered during each committee<br />
meeting:<br />
a. Previous month’s action items.<br />
b. Workplace safety inspection outstanding issues.<br />
c. Review incidents.<br />
d. Review employee suggestions.<br />
3. QUARTERLY WORKPLACE SAFETY INSPECTIONS — <strong>The</strong> HSE committee must ensure quarterly<br />
workplace inspections occur. At a minimum, they must:<br />
a. Document the inspection results.<br />
b. Recommend how to eliminate hazards and unsafe work practices in the workplace.<br />
c. Track non-compliances until completion.<br />
4. TRAINING—All HSE committee members must be trained in the following:<br />
• Purpose and operation of the HSE committee.<br />
• HSE Committee procedures.<br />
• Methods of conducting HSE committee meetings.<br />
• How to access all regulatory and Nike HSE standards that apply to the particular facility.<br />
• Hazard identification in the workplace.<br />
• Conducting effective accident and incident investigations.<br />
5. DOCUMENTATION<br />
Training Records: Each facility must maintain training records for a minimum of 3 years.<br />
<strong>Inc</strong>ident Records: Each facility must maintain incident records for a minimum of 5 years.<br />
