c. Documented listing of qualified contractors.<br />

3. PREJOB REVIEW/ORIENTATION—Each facility must conduct a contractor/subcontractor pre-job<br />

review and orientation which includes as a minimum:<br />

a. Orientation of facility including emergency exits, alarm recognition, and actions to take in the<br />

case of an emergency.<br />

b. Verification of any required contractor/subcontractor training and or certifications.<br />

c. Verification of material safety data sheet (MSDS) for any chemicals brought on site.<br />

d. Review of the equipment brought on site to ensure it is in good condition and complies with all<br />

regulatory requirements.<br />

e. Review of all applicable HSE regulations as well as facility HSE policies and procedures.<br />

f. Review of general safety rules.<br />

g. Housekeeping, cleanup and disposal requirements.<br />

h. <strong>Inc</strong>ident reporting.<br />

i. Provisions of noncompliance.<br />

4. MONITORING—Each facility must have a monitoring process for contractors and subcontractors.<br />

Level of monitoring should be determined by level of risks involved with tasks.<br />

5. PROVISIONS OF NONCOMPLIANCE—Each facility must have a process for noncompliance with any<br />

part of the contractor policy and procedures.<br />

6. TRAINING —All affected managers, supervisors and employees must be trained on the procedures<br />

of the contractor safety program.<br />

7. DOCUMENTATION—<br />

Training Records: Each facility must maintain training and pre-job review/orientation records for a<br />

minimum of 3 years.<br />

Qualification Records<br />

a. Each facility must maintain current prequalification/qualification forms.<br />

b. Each facility must maintain current evaluations of prequalification/qualification forms.<br />

Other Records<br />

a. Each facility must maintain current risk assessment of contractor tasks.<br />

b. Each facility must maintain monitoring records for a minimum of 3 years.<br />
