b. SMART (Specific, measurable, achievable, realistic and time bound) objectives. Plan may be<br />
combined or separate from overall business plan for facility.<br />
c. Assigned an owner responsible for implementation.<br />
3. DOCUMENT CONTROL—Each facility must have developed and implemented procedures to control all<br />
HSE related documents to ensure:<br />
a. Documents are legible with dates (of revision).<br />
b. Current revisions of documents can be located when needed.<br />
c. Documents are reviewed by authorized individuals at least every two years or when any<br />
significant changes occur.<br />
d. Obsolete documents are promptly removed from points of use to assure against unintended use.<br />
4. AUDIT—Each facility must have implemented an audit process to evaluate all aspects of their HSE<br />
management system at a time interval dependent upon overall risk of facility.<br />
5. NONCOMPLIANCE—Each facility must have developed and implemented procedures for identifying,<br />
prioritizing, investigating and resolving noncompliance with any aspect of the HSE System. Procedures<br />
must include at a minimum:<br />
a. Methods for assigning responsibility for activities.<br />
b. Description of actions required resolving and preventing noncompliance.<br />
c. Date actions must be completed by.<br />
d. Date of completion.<br />
6. MANAGEMENT REVIEW—Each facility must have developed and implemented procedures for an<br />
annual review of the HSE management system. At a minimum, management review procedures must be<br />
conducted annually and include:<br />
a. Progress against HSE strategic plan.<br />
b. Roles and responsibilities.<br />
c. Implementation of processes and procedures.<br />
d. Review of the HSE policy (every two years).<br />
e. Review of any audit results, recommendations, noncompliances and corrective and<br />
preventive actions.<br />
f. Review of performance metrics.<br />
g. Evaluation of system suitability, adequacy and effectiveness.<br />
7. COMMUNICATION—Each facility must properly inform employees about HSE. As a minimum, they must:<br />
