LASER SAFETY<br />

g. LASER systems must be calibrated and tested per manufacturer’s recommendations.<br />

h. All LASER system deficiencies must be corrected prior to operation.<br />

3. TRAINING—<br />

LASER safety awareness: Affected employees must receive awareness level training at the time of<br />

initial assignment. Training must cover as a minimum:<br />

• Affects of LASER radiation and specific hazards to which employees may be exposed<br />

and how those hazards are controlled.<br />

• Safe work practices.<br />

• Emergency Procedures.<br />

LASER safety: Authorized employees must receive initial training and annually thereafter. Training<br />

must cover as a minimum:<br />

• Type of LASERs present at the facility.<br />

• Potential hazards of exposure to LASERs present at the facility.<br />

• Exposure levels and resulting risks.<br />

• Results of hazard evaluations.<br />

• Safe work practices.<br />

• Emergency procedures.<br />

4. DOCUMENTATION—<br />

Training Records: Each facility must maintain training records for a minimum of 3 years.<br />

Medical Records: Each facility must maintain confidential and secure medical records for a<br />

minimum length of employment plus 30 years. Medical records shall not be disclosed without<br />

employee’s written consent, except as required by law.<br />

<strong>Inc</strong>ident Records: Each facility must maintain incident records for a minimum of 5 years.<br />

Other Records<br />

a. Current risk assessment.<br />

b. Calibration records for testing equipment for a minimum of 3 years.<br />

c. Maintenance records for the life of equipment.<br />

Except where specifically identified as a recommended practice, this Code Leadership Standard sets minimum<br />

standards – contractors must comply with any applicable higher legal requirement and are encouraged to<br />

continue to develop their own practices which provide greater protection for their employees.<br />
