OCCUPATIONAL NOISE EXPOSURE<br />
a. No cost to employees.<br />
b. Conducted by a certified medical professional.<br />
c. Testing (audiogram) conducted initially and annually.<br />
d. Notification to affected employees of results.<br />
e. Follow up/corrective action with any change in hearing as identified by the certified medical<br />
professional.<br />
4. TRAINING—Training will be conducted when a facility has a hearing conservation program for all<br />
employees at the time of initial assignment and at least annually thereafter. Training must cover as<br />
a minimum:<br />
• Effects of noise on hearing.<br />
• Purpose of hearing protectors.<br />
• Advantages, disadvantages, and attenuation of various types.<br />
• Instructions on selection, fitting, use, and care.<br />
• Purpose of audiometric testing and an explanation of the testing process.<br />
5. DOCUMENTATION<br />
Training Records: Each facility must maintain training records for a minimum of 3 years.<br />
Medical Records: Each facility must maintain confidential and secure medical records for a<br />
minimum length of employment, plus 30 years. Medical records shall not be disclosed without<br />
employee’s written consent, except as required by law.<br />
<strong>Inc</strong>ident Records: Each facility must maintain incident records for a minimum of 5 years.<br />
Other Records<br />
a. Current risk assessment.<br />
b. Noise assessment measurements will be retained for a minimum of 5 years.<br />
Except where specifically identified as a recommended practice, this Code Leadership Standard sets minimum<br />
standards – contractors must comply with any applicable higher legal requirement and are encouraged to<br />
continue to develop their own practices which provide greater protection for their employees.<br />
