INJURY/ILLNESS SYSTEM MANAGEMENT<br />
• Provides opportunity for ‘employee participation’ a basic requirement for successful HSE<br />
Program. This embodies principles of behavior shift, responsibility sharing, awareness, and<br />
incentives etc.<br />
• One of the primary workplace problems Near Miss incident reporting attempts to solve,<br />
whether directly or indirectly, is to try and create an Open culture whereby everyone<br />
shares and contributes in a responsible manner. Near-Miss reporting has been shown to<br />
increase employee relationships and encourage teamwork in creating a safer work<br />
environment.<br />
3. TRAINING—<br />
Injury and Illness Reporting: employees must be trained on the location’s injury and illness<br />
management program. Training must include as a minimum:<br />
• Immediate reporting of any work related injury or illness or near miss regardless of the<br />
severity.<br />
• Communication of information regarding any injury or illness that affects their ability to<br />
perform normal work duties.<br />
Injury and Illness Management: managers and supervisors must receive additional training<br />
regarding the location’s Injury and Illness and Near Miss Management Program. Additional training<br />
must include as a minimum:<br />
• Processing a report of injury or illness or near miss.<br />
• Conducting accident or near miss investigation/root cause analysis.<br />
• Maintaining confidentiality.<br />
• Communications with employee, medical personnel, and other stakeholders.<br />
• Returning employee to work after injury/illness.<br />
4. DOCUMENTATION—<br />
Training Records: Each facility must maintain training records for a minimum of 3 years.<br />
Medical Records: Each facility must maintain confidential and secure medical records for a<br />
minimum length of employment plus 30 years. Medical records shall not be disclosed without<br />
employee’s written consent except as required by law.<br />
<strong>Inc</strong>ident Records: Each facility must maintain incident records for a minimum of 5 years.<br />
Other Records: Injury and illness logs must be maintained for 5 years following the end of the year to<br />
which they are related.<br />
Except where specifically identified as a recommended practice, this Code Leadership Standard sets<br />
minimum standards – contractors must comply with any applicable higher legal requirement and are<br />
encouraged to continue to develop their own practices which provide greater protection for their employees.<br />
