INJURY/ILLNESS SYSTEM MANAGEMENT<br />

• Name and number of fatalities or hospitalized employees.<br />

• Contact person and phone number.<br />

• Complete description of the incident and all contributing causes.<br />

• Corrective measures are necessary to prevent recurrence.<br />

d. Injury/illness management:<br />

• <strong>Inc</strong>ident confidentiality.<br />

• Communication with injured employee (e.g. wages, medical restrictions, etc.).<br />

• Return to work provisions (including any work restrictions and transitional work).<br />

• Enforcement of any work restrictions.<br />

2. RECORDKEEPING —Each facility must keep a record of all work related injuries and illnesses resulting<br />

in a fatality, hospitalization, lost workdays, medical treatment beyond first aid, job transfer or<br />

termination, or loss of consciousness for that factory which includes:<br />

a. Each event entered no later than 6 working days after receiving the information.<br />

b. Name of employee or contractor.<br />

c. Date of injury or illness.<br />

d. Where injury or illness occurred.<br />

e. General description of accident.<br />

f. Number of restricted calendar days of work due to injury or illness.<br />

g. Number of calendar days away from work due to injury or illness.<br />

h. Annual summary of injuries/illnesses must be posted in areas accessible to employees. This must<br />

include:<br />

• Total number of injuries and illnesses events.<br />

• Total number of deaths.<br />

• Total number of days away from work.<br />

• Total number of cases restricted work activity or job transfer.<br />

• <strong>Inc</strong>ident rate, which is calculated as follows:<br />

- (Total number of days away from work + total number of cases with job transfer or<br />

restriction) X 200,000 / Number of hours worked by all employees = Total <strong>Inc</strong>ident Rate.<br />

i. Additionally, each facility must capture, track and summarize each near miss.<br />

• Near misses are smaller in scale, relatively simpler to analyze and easier to resolve.<br />

• Captures sufficient data for statistical analysis; trending studies.<br />
