INJURY/ILLNESS SYSTEM MANAGEMENT<br />
STANDARD<br />
<strong>The</strong> contractor provides a safe, hygienic and healthy workplace setting and takes necessary steps to<br />
prevent accidents and injury arising out of, linked with or occurring in the course of work or as result of<br />
the operation of contractor’s facilities. <strong>The</strong> contractor has systems to detect, avoid and respond to<br />
potential risks to the safety and health of all employees.<br />
‣ Develop and implement processes and procedures for incident reporting and injury and illness<br />
management. Contractor must comply with requirements as outlined in this standard or<br />
relevant local laws and regulations, whichever is more stringent.<br />
RESPONSIBILITIES<br />
Location Manager must ensure that an injury and illness management program is developed,<br />
implemented and followed.<br />
HSE Representative must establish, maintain and administer the injury and illness management program<br />
and procedures.<br />
Managers and Supervisors must ensure that employees are trained and adhere to the requirements of<br />
the injury and illness management program and procedures.<br />
Employees must adhere to the requirements of the injury and illness management program and<br />
procedures.<br />
DEFINITIONS<br />
• Work Related Injury or Illness is an event or exposure in the workplace that either caused or<br />
contributed to an injury or illness or aggravated a preexisting injury or illness.<br />
• Near Miss is an unplanned event that did not result in injury, illness, or damage - but had the<br />
potential to do so.<br />
REQUIREMENTS<br />
1. POLICIES & PROCEDURES—Each facility must implement procedures to manage injury and illness<br />
which must cover, as a minimum, the following:<br />
a. All incidents (work related injuries, illnesses, accidents resulting in property damage or near<br />
misses) must be reported immediately to management.<br />
b. All fatalities or serious injuries (e.g. incidents resulting in 24 hour inpatient hospitalization,<br />
permanent disfigurement, loss of any body part or loss of sight, etc.) must be communicated to<br />
a Nike, <strong>Inc</strong>. representative within 8 hours of incident occurrence.<br />
c. <strong>Inc</strong>ident investigation report must be submitted to location management within 48 hours. <strong>The</strong><br />
report must include as a minimum:<br />
• Name of site location.<br />
• Specific location and time of the incident.<br />
• Relevant facts and witness information.<br />
