TEAM PRE-REQUISITES:
• All players must be between 13-17 years old and their date of birth must be on or after 1 st January 1997
and not after 31 st December 2000
• Only boys can participate in the boys team and only girls can participate in the girls team.
Mixed teams are not allowed
• Each team should have a minimum of 16 players per team and a maximum of 18 players per team.
In the event of an injury or inability to travel, Teams are able to introduce replacements who must
also meet the above criteria.
TEAM REQUIREMENTS/SUBMISSIONS
• Each team to submit fully completed Registration Kit including:
- Completed Team Registration Form
- Team Questionnaire
- Copy of Coach’s Original Identification Card
- Copy of Manager ID (Optional if team has a Manager)
- Completed Individual Player Registration Kit for each Team Member
• Completed Player Registration Form
• Rules of Participation signed by Parent/Guardian (Each page to be initialed)
• Copy of original birth certificate/baptismal card signed by parent/legal guardian
• 2x recent passport size photos
• Copy of parents/guardians original Identification Card
• Complete Registration Kit to be received no later than ____________ 2013
• Completed Registration Kits to be dropped to any Airtel Owned Shop or sent via Post to the
