Checklist of Eligibility and Technical Component for
Bidders
The Eligibility and Technical Component shall contain one (1) Original and Two (2) Duplicate
Copies of the following:
ELIGIBILITY DOCUMENTS
Class A Documents
Registration certificate from SEC, Department of Trade and Industry (DTI) for sole
proprietorship, or CDA for cooperatives ;
Mayor’s permit issued by the city or municipality where the principal place of business
of the prospective bidder is located;
Valid Philippine Contractors Accreditation Board (PCAB) license and registration for the
type and cost of the contract to be bid (Sample Form No. SF-INFR-09).
Statement of all its on-going government and private contracts including contracts
awarded but not yet started reckoned from the submission of bids , supported with
any of the following: Notice of Award, Notice to Proceed issued by the Owner
(Sample Forms No. SF-INFR 15 ).
Statement of similar completed government and private contracts within ten (10)
years from submission of bids, the statement shall be supported by the Constructors
Performance Evaluation System (CPES) rating sheets, and/or certificates of completion
and owner’s acceptance, if applicable (Sample Forms No. SF-INFR 16).

Audited financial statements, showing, among others, the prospective total and
current assets and liabilities, stamped “received” by the BIR or its duly accredited and
authorized institutions, for the preceding calendar year which should not be earlier
than two (2) years from the date of bid submission.
Contracting Capability in accordance with ITB Clauses 5.5.
Any one of the Following:
o Computation of Net Financial Contracting Capacity (NFCC) which must be equal
to ABC (Sample Form No. SF-INFR 19) , OR
o Certificate of commitment specific to the contract to be bid, issued by a
Universal or Commercial bank as authorized by the Bangko Sentral ng Pilipinas
(BSP) to issue such financial instrument, to extend to the bidder a credit line if
awarded the contract which shall at least equal to 10% of ABC. (Sample Form
No. SF-INFR 20)
Class B Documents
If applicable, valid Joint Venture Agreement (JVA) or, in lieu thereof, duly notarized
statements from all the potential joint venture partners stating that they will enter
into and abide by the provisions of the JVA in the instance that the bid is successful
shall be included in the bid.

Bid Security
Any one of the following:
Cash, cashier’s/manager’s check confirmed by a Universal or Commercial
Bank in the amount of Php 1,745,099.78 (2% of ABC); or
Bank Draft/Guarantee or irrevocable letter of credit issued by a Universal or
Commercial bank in the amount of Php 1,745,099.78 (2% of ABC)
Provided, however, that it shall be confirmed or authenticated by a Universal
or Commercial bank, if issued by a foreign bank (Sample Form No. SF -INFR-
36), and
Surety Bond in the amount of Php 4,362,749.45 (5% of ABC)
(Sample Form No. INFR-38 for GSIS Bond)
Except for the following CDC blacklisted Insurance Company:
Sterling Insurance
Stronghold Insurance
Prudential Guarantee and Insurance Corporation
Any combination of the foregoing
Organizational chart for the contract to be bid;
List of contractor’s personnel (viz, project Manager, Project Engineers, Electrical
Engineer, Materials Engineers, and Foremen), to be assigned to the contract to be
bid, with their complete qualification and experience data;
List of contractor’s equipment units, which are owned, leased, and/or under purchase
agreements, supported by certification of availability of equipment from the
equipment lessor/vendor for the duration of the project; and
Duly Notarized and Accomplished Omnibus Sworn Statement ( Section IX Bidding
Forms page 121)
Duly Signed Bid Bulletins
Checklist of Financial Component for Bidders
Financial Envelope shall contain one (1) Original and Two (2) Duplicate Copies of the following:
Financial Bid Form in accordance with the form prescribed in Section IX Bidding
Forms;
Cost Summary;
Detailed Bill of Quantities in accordance with the form prescribed in Section VIII
including Unit Price Analysis for each item of works (Unit prices of Construction
Materials, Labor rates and Equipment rentals used);
Day Work Schedule; and
Duly Signed Cash Flow and Payment Schedule
