The NATIONAL FOOD AUTHORITY (NFA), through its Corporate Budget for
2013 intends to apply the sum of FOUR HUNDRED SEVENTY TWO THOUSAND
FIVE HUNDRED PESOS(P472,500.00) for the procurement of pre-cut lumbers for
the fabrication of five hundred (500) pieces of wooden pallets in NFA Southern
Leyte .Quotations received in excess of the ABC shall be automatically rejected at
bid opening.
The NFA now invites interested parties to quote for the hereunder contract:

To expedite the implementation of the project, said activity will be conducted through
emergency purchase as specified in the Revised Implementing Rules and
Regulations (IRR) of the Republic Act 9184 (RA 9184) otherwise known as the
“Government Procurement Reform Act”.

Interested parties may submit their SEALED QUOTATIONS on August 20,
2013 at 2:00 P.M. at NFA-8 Conference Room, Pawing, Palo, Leyte.
Other information can be obtained from the NFA and inspect/purchase the
Bidding Documents at the address given below from 8:00 A.M. To 5:00 P.M. upon
payment of the non-refundable fee as specified above.
Bidders shall submit four (4) certified photocopies of the following legal
documents provided that its original documents shall be authenticated by the
concerned NFA provincial manager or his authorized representative before the
submission and opening of sealed quotations on August 20, 2013 at 2:00 P.M and
2:30 P.M. respectively, at NFA-8 Conference Room, Pawing, Palo, Leyte, to wit;
1. Dept. of Trade and Industry (DTI) permit for the sole Prop/SEC
for Partnership and Corporation
2. Valid and Current Mayor’s Permit
3. VAT Registration or BIR Certificate of Registration which
contains Taxpayer’s Identification Number (TIN)
4. Valid DENR Registration/License
5. Duly notarized Omnibus Sworn Statement
