Applicant’s declaration and consent<br />

I declare that all data provided by me is true and that I have not concealed any significant facts. In accordance<br />

with Article 7 of Act No. 363/2005 Coll. on Protection of Personal Data I give consent with my own autograph that<br />

the university can process my personal data for the purposes of admission proceedings until the date of<br />

registration to the first year of the PhD study at the university. This consent relates to personal data stated in the<br />

application form and its appendices, which the university is eligible to ask for in accordance with Article 58 Section<br />

5 of Act No. 131/2002 Coll. on Universities and on Amendments of Some Acts as amended by later regulations<br />

(hereinafter referred to as ”act”). I take into account that I can cancel this consent in writing only in the relevant<br />

university or faculty if there occur any relevant reasons. At the same time I take into account that the data can be<br />

processed only by authorized persons who are obliged to observe regulations of Act No. 363/2005 Coll. on<br />

Protection of Personal Data, and the processed data will be archived and discarded in accordance with effective<br />

legal regulations of the Slovak Republic.<br />

Instructions and explanatory notes for filling out the application:<br />

The application is intended for second-degree university study graduates.<br />

1) Check appropriate box in the Sex item.<br />

2) In the <strong>Form</strong> of study item enter ”full-time” or ”external” according to information about possibilities of the study.<br />

3) In the Method of study item enter ”non-distance” or ”distance”, or ”combined” according to information about<br />

possibilities of the study.<br />

4) In case printed data is not sufficient, write it on a separate appendix. Specify also the study that you duly<br />

graduated from, current study or incomplete study (for example completed semesters).<br />

Applicants with physical disability can ask for another form of entrance examination and the way how it should be<br />

carried out with regard to their physical disability.<br />

Notes to the “Applicant’s declaration and consent” section:<br />

This consent relates to specific data connected for example with further conditions for admission to study, which<br />

the act cannot specify with regard to heterogeneity of requirements of universities regarding the field of study (for<br />

example data on health qualification) – see Article 58 Section 5 of the Act.<br />

This consent does not relate to for example first name, surname, personal identification number, place of birth,<br />

marital status, etc., and therefore the consent of the person in question is not required for processing of this data<br />

– see Article 58 Section 3 of the Act, where the data is listed. The consent relates to all other data.<br />

Appendices to the application form:<br />

1. curriculum vitae,<br />

2. copy of the pay receipt for admission proceedings,<br />

3. verified copies of documents proving graduation from study (university diploma, state-examination<br />

certificate, diploma supplement),<br />

4. list of published scholarly and scientific (artistic) works,<br />

5. confirmation of practical experience, this relates to candidates applying for full-time form of study,<br />

6. project outline of the dissertation thesis topic.<br />
