4.0 Inspections<br />

The permittee shall conduct inspections in accordance with Parts 4.1, 4.2, and 4.3 of this permit<br />

at the facility. If, during any quarterly routine facility inspection, visual assessment, or comprehensive<br />

facility inspection, or any other time, the facility’s control measures are found to be inadequate or<br />

otherwise not be properly operated and / or maintained, the permittee shall review the selection, design,<br />

installation, and implementation of the control measures to determine if maintenance and/or modifications<br />

are necessary to meet the applicable numeric effluent limitations in Part 2.2.1 and water quality-based<br />

requirements in Parts 2.2.2 and 2.2.3 of this permit, in accordance with the requirements of Part 2.1.1.3.<br />

Such modifications shall be documented in the SWPPP and implemented as expeditiously as practicable.<br />

Additional sector-specific inspection requirements may be described in Part 8 of this permit. If a<br />

conflict exists between the two, the requirements of Part 8 shall prevail.<br />

4.1 Routine Facility Inspections<br />

4.1.1 Routine Facility Inspection Procedures<br />

The permittee shall conduct routine inspections of all areas of the facility where industrial<br />

materials or activities are exposed to stormwater with the potential to discharge from the facility, and of all<br />

stormwater control measures used to comply with this permit. Such routine inspections shall be<br />

conducted at least once each calendar quarter beginning with the first full calendar quarter after the<br />

facility becomes covered under this permit (see Part 1.3.1(2) and Table1-2). More frequent inspections<br />

(e.g., monthly) may be appropriate for some types of equipment, processes, and control measures or<br />

areas of the facility with significant activities and materials exposed to stormwater. The permittee shall<br />

specify the relevant inspection schedules in the SWPPP document as required in Part 5.1.5.<br />

A qualified person or persons (see definition in Appendix A) shall conduct routine facility<br />

inspections. A member of the stormwater pollution prevention team (see Part 5.1.1) shall conduct or<br />

participate in the inspections. Inspections shall be performed during periods when the facility is in<br />

operation (i.e., is not inactive and unstaffed in accordance with the requirements of Part 4.1.3). The<br />

permittee shall initiate at least one of the routine facility inspections each calendar year while a<br />

stormwater discharge is occurring at one or more outfalls, but in no case later than 24 hours or the first<br />

business day (whichever comes later) following the end of the measurable storm event.<br />

If there is no measurable storm event(s) during a calendar year, the permittee shall document the<br />

inability to perform an inspection during a measurable storm event as described in Part 5.4. In any case,<br />

the permittee must still complete routine quarterly inspections.<br />

4.1.2 Routine Facility Inspection Documentation.<br />

The permittee shall document the findings of each routine facility inspection performed and<br />

maintain this documentation with the SWPPP as required in Part 5.4. Inspection findings do not need to<br />

be submitted to ADEQ, unless specifically requested. At a minimum, the documentation for each routine<br />

facility inspection must include:<br />

The inspection date and time;<br />

The name(s) and signature(s) of the inspector(s);<br />

Weather information and a description of any discharges occurring at the time of the<br />

inspection;<br />

Evidence demonstrating that previously unidentified discharges of pollutants have occurred<br />

from the site;<br />

Any control measures needing maintenance or repairs;<br />

Any failed control measures that need replacement;<br />

Any other evidence of deviations from the permit or SWPPP observed; and<br />

Any additional control measures needed to comply with the permit requirements.<br />
