Corrective Action Report<br />

As required by MSGP-2010, Part 3.3, within 72 hours of discovery of any conditions listed in<br />

Part 3.1, the permittee shall identify the condition triggering the need for corrective action<br />

review, a description of the problem identified and the date the problem was identified, which<br />

shall be maintained with this SWPPP. In addition, within 14 calendar days of discovery of any<br />

condition listed in Part 3.1, the permittee shall summarize the corrective action taken or to be<br />

taken, whether SWPPP modifications are required as a result of this discovery or corrective<br />

action, the date the corrective action was initiated or will be initiated and the date the corrective<br />

action was completed or is expected to be completed, which shall be maintained with the<br />

SWPPP.<br />

While conducting visual assessments at Outfalls 15 and 16, inspectors<br />

observed white flakes in the discharge.<br />

Summarize Corrective Action:<br />

Consultant performing visual monitoring assessment inspected the contributing areas leading to Outfalls<br />

15 and 16. The white particles were traced back to a large section of new concrete located immediately<br />

south of the Southwest Maintenance Hangar and wash bay. Consultant observed identical white particles<br />

in the concrete cracks, along the curb, and in a storm drain inlet. Particles appeared to be wide-spread<br />

and apparently not attributable to a single point source.<br />

Consultants made contact with a Southwest employee. The employee stated that the particles are from a<br />

sealant that was applied to the concrete approximately one year ago and that it flakes up occasionally.<br />

The employee organized a street sweeper to clean the pavement and curbing. Consultant notified City of<br />

<strong>Phoenix</strong> Aviation personnel and the Southwest <strong>Pollution</strong> <strong>Prevention</strong> Team Member of the issue and<br />

follow up activities.<br />

Southwest will increase street sweeping in the area, especially before rain events. City of <strong>Phoenix</strong><br />

Aviation personnel will visit the area later in the day to confirm adequate street sweeping was performed.<br />

This area will be revisited during the January Comprehensive Facility Inspection.<br />

No changes to the SWPPP are planned in response to this incident.
