Lavatory Waste Release Response Report March 19, 2013<br />

BACKGROUND<br />

On Sunday March 17, 2013, Julie Riemenschneider received e-mail notification from the Operations<br />

Department regarding a potential lavatory spill/leak near the US Airways Maintenance Hangar at <strong>Sky</strong> <strong>Harbor</strong><br />

Airport. Julie forwarded the notification on the morning of Monday, March 18, 2013 to Lisa Farinas who<br />

then forwarded the information to CDM-Smith and EEC on Monday at approximately 10:35am.<br />

Lisa Farinas surveyed the area and provided the following initial photographs of the incident:<br />

EEC FOLLOW-UP ACTIVITIES<br />

Upon notification, EEC contacted the US Airways <strong>Pollution</strong> <strong>Prevention</strong> Team (PPT) Member to determine<br />

what (if any) association US Airways had with the incident. The PPT Member responded by meeting with<br />
