PROJECT ACTIVITIES<br />

Site Safety <strong>Plan</strong> - Prior to performing site activities, Four Corners developed a Site Safety <strong>Plan</strong> (SSP) for<br />

the subject property to outline the scope of work and the potential health risks involved with the work<br />

activities. The SSP reviewed potential on-site physical hazards, chemical hazards, monitoring procedures,<br />

emergency procedures, and evacuation routes. Four Corners personnel who were on-site during the<br />

investigative activities were required to review and sign the SSP prior to commencing the field activities.<br />

Additionally, Four Corners attended one meeting on September 24, 2012 as requested by the City of<br />

<strong>Phoenix</strong> to discuss and coordinate field activities.<br />

Remediation Oversight - Four Corners conducted fuel impacted soil remediation oversight on September<br />

24, 2012. Four Corners inspected soils along the southern bank of the Salt River channel to determine if<br />

signs of fuel impacts, such as soil staining or odors, were present that would indicate a need for soil<br />

remediation. Signs of fuel impacts were identified for approximately 0.3 mile long portion of a dirt<br />

roadway on the southern bank of the Salt River channel, with a large stain on the western terminus of the<br />

roadway stains, where the grader had stopped. The locations of soil staining is illustrated on Figure 1, Soil<br />

Sample Locations, included in Attachment A, Figure. The City of <strong>Phoenix</strong> retained internal personnel to<br />

conduct remediation activities, including using absorbent pads and materials, and various types of<br />

excavation equipment as required for the project. Four Corners personnel observed the remediation of<br />

impacted soil. Four Corners mapped the potential extent of the fuel impacts along the southern bank of the<br />

Salt River channel and documented where stains or fuel-impacted soils were identified. Four Corners’<br />

observations are retained in the project file.<br />

Roadway staining was scraped for 0.3 miles and the stain observed on the western end of the impacted<br />

roadway soil was excavated. The final excavation pit was approximately 15' x 75' x 5'. Approximately<br />

300 tons of contaminated soil was removed.<br />

Sampling Activities<br />

Confirmation Sampling - At the completion of remediation activities, Four Corners conducted a visual<br />

survey of the excavation zone for remaining soil staining and collected soil confirmation samples to provide<br />

documentation of completion of remediation activities. Prior to submittal to the analytical laboratory, Four<br />

Corners composited soil samples from stained soil along the roadway on the southern bank of the Salt<br />

River channel from sample locations 12087-S1, 12087-S1A, and 12087-S1B into 12087-S1-Comp; 12087-<br />

S2, 12087-S2A, and 12087-S2B into 12087-S2-Comp; and 12087-S3, 12087-S3A, and 12087-S3B into<br />

12087-S3-Comp. However, soil samples collected for VOC analysis cannot be composited, and the<br />

samples were collected from a representative location of the roadway as determined by field personnel<br />

at a depth of approximately half an inch below the surface. Six samples were collected from the excavation<br />

zone (12087-EX@3.5', 12087-EX@5', 12087-NW@3', 12087-E@4', 12087-SE@2.5', and 12087-W@3')<br />

on the western terminus of the roadway staining. Twelve soil samples were submitted to the laboratory.<br />
