Section 7 - Reporting and Recordkeeping<br />

7.1 Annual Report<br />

As required by MSGP-2010 Part 7.2, PECM or its stormwater consultant prepares an<br />

Annual Report for the annual reporting period from June 1 to May 31. The report<br />

includes:<br />

• Findings from the CFIs;<br />

• Corrective Action Forms and Reports;<br />

• Visual assessment results; and<br />

• The results of monitoring required by ADEQ.<br />

Copies of the Annual Reports are uploaded to the stormwater database and are available<br />

with the SWPPP as Attachment 13, as required by MSGP-2010 Part 7.2. The airport does<br />

not discharge to an impaired or OAW stream and is not required to submit the Annual<br />

Report to ADEQ, according to MSGP-2010 Part 7.2. The Annual Report is completed by<br />

July 15 each year for the prior reporting period.<br />

7.2 Other Reporting<br />

7.2.1 24-Hour Reporting<br />

The MSGP-2010 Appendix B part 12(d) requires PHX to report permit noncompliance<br />

issues that may endanger human health or the environment. Within 24-hours following<br />

such a noncompliance event, PECM verbally notifies ADEQ at:<br />

7.2.2 Five-Day Follow-Up Reporting to the 24-Hour Reporting<br />

Within five days of an event triggering a 24-hour reporting, PECM will provide ADEQ<br />

with a written submission. A five-day follow-up report will be submitted if the<br />

discharge exceeds effluent limitation in the permit or if the maximum daily discharge<br />

limitation is violated for a pollutant listed in the permit. Written submissions will<br />

include:<br />

• Exact date and time of the incident and its duration;<br />

• A description of the noncompliance issue and its cause;<br />

• If the noncompliance issue has not been corrected, then the anticipated timeframe for<br />

when the noncompliance issue will be resolved; and<br />

• Steps taken or planned to reduce, eliminate, and prevent recurrence.<br />
