• The permittee becomes aware or ADEQ determines that the facility’s discharge<br />

causes or contributes to an exceedance of an applicable water quality standard; and<br />

• ADEQ determines that modifications to the control measures are necessary to meet<br />

the requirements of the MSGP-2010 Part 2.2.<br />

Within 72 hours of identifying one of these corrective action triggers, PECM will<br />

document the discovery with the following information on the Corrective Action Form:<br />

• Identification of the triggering condition<br />

• Description of the problem identified<br />

• Date the problem was identified<br />

Within 14 calendar days, PECM will document the following information in a Corrective<br />

Action Report:<br />

• Summary of corrective action(s) taken or to be taken<br />

• Whether SWPPP modifications are required as a result of the discovery or corrective<br />

action<br />

• Date corrective action was or will be initiated<br />

• Date the corrective action was completed or is expected to be complete<br />

Corrective Action Forms and Corrective Action Reports (Attachment 12) will be<br />

uploaded to the stormwater database and are available with the SWPPP as required by<br />

MSGP-2010 Part 3.3.<br />
