A Southwest Airlines 737 commercial aircraft<br />

allowed the release of 15 gallons (20 x 20) of<br />

Jet A fuel due to a malfunctioning emergency<br />

fuel shut-off valve. An estimated 1-2 gallons<br />

reached the nearby storm drain.<br />

Early Thursday morning July 11, 2013 the<br />

large striper (Eq, No. 528003) spilled<br />

approximately 120 gallons of white paint at the<br />

Facilities yard striping cage. The cause was a<br />

2" hose that loosened from the pipe fitting,<br />

likely due to faulty clamps.<br />

F3 and Operations responded for a Jet A fuel<br />

spill.<br />

Small diesel fuel leak occurred while refueling<br />

ground support equipment.<br />

an auto shutoff valve system malfunction.<br />

Swissport Fueling 8/7/2013 T4 B22 Fuel 30 x 15 Fuel spill of unknown amount at T4 B22.<br />

Fuel released to concrete was<br />

collected with clay absorbent. Fuel<br />

released to the concrete trench drain<br />

was vacuumed, and the drain was<br />

then cleaned out (with all fluids<br />

collected). No material reached the<br />

outfall. Environmental confirmed the<br />

fuel did not reach the south river<br />

bottom.<br />

Fleet has replaced these clamps with<br />

heavier clamps that are more<br />

appropriate for the hose. Absorbent<br />

was applied and material was dumped<br />

into the used absorbent bin.<br />

O-335, O-331, and F3 responded.<br />

Swissport cleaned up the spill and<br />

removed the cart.<br />

Operations cancelled F3 because the<br />

spill was only 2 gallons. Piedmont<br />

cleaned up the spill.<br />

Operations responded and the spill<br />

was contained and cleaned by<br />

Swissport.<br />

American Airline personnel cleaned<br />

the spill.<br />

O-330 and O-310 responded. Spill<br />

was contained<br />

O-310 and Landside Maintenance<br />

responded.<br />

Maintenance responded. The spill<br />

was contained and absorbent put<br />

down.<br />
