faulty high-level shutoff valve and inaccurate<br />

fuel gauges.<br />

Minor fuel spill on ramp.<br />

Approximately 40 x 20 spill caused by a faulty<br />

shutoff valve on aircraft N764SW.<br />

A leveling valve malfunction on <strong>Sky</strong>West RJ<br />

(N889AS) caused a small fuel spill.<br />

Fuel spilled from the right wing of a DHL 767<br />

while being fueled by Swissport fuel truck<br />

LSG <strong>Sky</strong> Chefs truck #7013 struck a bag cart<br />

at A4 damaging the hydraulic tank resulting in<br />

a spill. Driver continued onto A1, leaving a trail<br />

of hydraulic fluid.<br />

The spill was contained and there was<br />

no exposure to the storm drains.<br />

to Southwest.<br />

The problem was immediately<br />

recognized, souce shutoff, contained,<br />

reported, and cleaned.<br />

F3 responded and DHL contained and<br />

cleaned the spill.<br />

Fire responded, Operations already<br />

on scene. Os 325 advised spill was<br />

already being cleaned up by<br />

Swissport. No storm drains affected.<br />

LSG Manager responded and COP<br />

Facilities conducted cleanup. Jesus<br />

Franco was given and NOV.<br />

O-345 responded and Southwest<br />

cleaned the spill.<br />

Operations and Police responded. Per<br />

Os 326 spill is contained and was<br />

cleaned up. No storm drains affected.<br />

O-326 responded and Swissport<br />

cleaned the spill.<br />
