Non-Mining MSGP 2010 General Permit<br />

including all calibration and maintenance records and all original strip chart recordings for<br />

continuous monitoring instrumentation, copies of all reports required by this permit, and<br />

records of all data used to complete the application for this permit, for at least three (3) years<br />

from the date this permit expires This period may be extended by request of the Director at<br />

any time. Permittees must submit any such records to ADEQ upon request. The permittee<br />

must retain the SWPPP developed in accordance with Part 5 of this permit, for at least three<br />

(3) years after the last modification or amendment is made to the plan.<br />

c. Records Contents. Records of monitoring information must include:<br />

i. The date, exact place, and time of sampling or measurements;<br />

ii. The initials or name(s) of the individual(s) who performed the sampling or measurements;<br />

iii. The date(s) analyses were performed;<br />

iv. The time(s) analyses were initiated;<br />

v. The initials or name(s) of the individual(s) who performed the analyses;<br />

vi. References and written procedures, when available, for the analytical techniques or<br />

methods used;<br />

vii. The analytical techniques or methods used; and<br />

viii. The results of such analyses.<br />

d. Approved Monitoring Methods. Monitoring must be conducted according to test<br />

procedures approved under 40 CFR 136, unless specific test procedures have been<br />

otherwise specified in this permit.<br />

e. Any person who falsifies, tampers with, or knowingly renders inaccurate any monitoring<br />

device or method required to be maintained in this permit is subject to the enforcement<br />

actions established under A.R.S. Title 49, Chapter 2, Article 4, which includes the possibility<br />

of fines and/or imprisonment.<br />

12. Reporting Requirements. [A.A.C. R18-9-A905(A)(3)(a) which incorporates 40 CFR 122.41(l)]<br />

a. <strong>Plan</strong>ned changes. The permittee shall give notice to the Director as soon as possible of<br />

any planned physical alterations or additions to the permitted facility. Notice is required only<br />

when:<br />

i. The alteration or addition to a permitted facility may meet one of the criteria for<br />

determining whether a facility is a new source in 40 CFR 122.29(b) (incorporated by<br />

reference at A.A.C. R18-9-A905(A)(1)(e)); or<br />

ii. The alteration or addition could significantly change the nature or increase the quantity of<br />

pollutants discharged. This notification applies to pollutants which are subject neither to<br />

effluent limitations in the permit, nor to notification requirements under 40 CFR<br />

122.42(a)(1) (incorporated by reference at A.A.C. R18-9-A905(A)(3)(b)).<br />

b. Monitoring reports. Monitoring results must be reported at the intervals specified<br />

elsewhere in this permit.<br />

i. Monitoring results must be reported on a Discharge Monitoring Report (DMR) or forms<br />

(paper or electronic) provided or specified by ADEQ. Pursuant to Part 7.1, all monitoring<br />

data collected pursuant to Part 6.2 and 6.3 must be submitted to the Department using<br />

the MSGP Discharge Monitoring Report (MDMR) form, available at<br />

http://www.azdeq.gov/environ/water/permits/stormwater.html .<br />

ii. If the permittee monitors any pollutant more frequently than required by the permit using<br />

test procedures approved under 40 CFR Part 136 unless otherwise specified in 40 CFR<br />

Part 503, or as specified in the permit, the results of this monitoring shall be included in<br />

the calculation and reporting of the data submitted in the DMR.<br />

iii. Calculations for all limitations which require averaging of measurements must use an<br />

arithmetic mean and non-detected results must be incorporated in calculations as the<br />

<strong>Stormwater</strong> Discharges Associated With Industrial Activity – Appendix B B-5
