6.3 Follow-up Actions if Discharge Exceeds Numeric Effluent Limit or a Water Quality Standard.<br />

The permittee shall conduct follow-up monitoring within 30 calendar days (or during the next<br />

qualifying runoff event, should none occur within the 30 days) of implementing corrective action(s) taken<br />

in accordance with Part 3 in response to an exceedance of a numeric effluent limit or water quality<br />

standard contained in this permit as described in Parts 2.2.1 and 2.2.2. Monitoring must be performed for<br />

any pollutant(s) that exceeds the effluent limit or water quality standard. If this follow-up monitoring<br />

exceeds the applicable effluent limit or water quality standard, the permittee shall comply with both Parts<br />

6.3.1 and 6.3.2.<br />

6.3.1 Submit an Exceedance Report.<br />

The permittee shall submit an Exceedance Report consistent with Part 7.3.<br />

6.3.2 Continue to Monitor.<br />

The permittee shall continue to monitor, at least twice per wet season, until the discharge is in<br />

compliance with the effluent limit or water quality standard or until ADEQ waives the requirement for<br />

additional monitoring.<br />

7.0 Reporting and Recordkeeping<br />

7.1 Reporting Monitoring Data to ADEQ<br />

7.1.1 The permittee shall submit monitoring data collected in accordance with Parts 4.2, 6.2, 6.3, and<br />

6.4 to ADEQ at the address in Part 7.6.<br />

7.1.2 Permittees shall use the MSGP discharge monitoring report (DMR) form available at<br />

http://www.azdeq.gov/environ/water/permits/stormwater.html<br />

7.1.3 The permittee shall compile all sampling results for the previous two wet seasons onto DMR<br />

form(s). Except as provided in Part 7.2 below, the permittee shall submit the DMRs to ADEQ not<br />

later than July 15 of each year of permit coverage.<br />

7.2 Annual Report<br />

All facilities shall prepare an Annual Report on a form provided by the Department and retain a<br />

copy of the report with the SWPPP. The Annual Report for the reporting period June 1 to May 31 shall be<br />

completed by July 15 and include, at a minimum:<br />

The findings from the facility’s Part 4.3 comprehensive facility inspection;<br />

Any corrective action documentation as required in Part 3.3;<br />

The DMR form(s) as required in Part 7.1 for the preceding two wet seasons; and<br />

The results of any monitoring required in Part 6.2 for those facilities that discharge to a<br />

water (or within 2.5 miles of a water if required by ADEQ) or portion thereof, classified as<br />

an OAW or an impaired water, or<br />

The results of any monitoring required in Part 6.2 if notified by the Department in<br />

accordance with Part 1.3.1(2)(c).<br />

Permittees with facilities that discharge to a water (or within 2.5 miles of a water if required by<br />

ADEQ, or is otherwise referenced in an approved TMDL) or portion thereof, classified as an OAW or an<br />

impaired water shall submit the annual report to ADEQ on or before July 15 (postmark date).<br />

7.3 Exceedance Report for Numeric Effluent Limitations or Water Quality Standards<br />

If follow-up monitoring pursuant to Part 6.3 exceeds a numeric effluent limit or water quality<br />

standard, the permittee shall submit an Exceedance Report to ADEQ no later than 30 calendar days after<br />

receiving the facility’s lab results. The facility’s Exceedance Report shall include the following:<br />
