Insurance Coverage for Surgery<br />
It is very important to notify your insurance company regarding your surgery. Confirm your policy is<br />
active. You need to check with the Benefits Department to verify your insurance benefits regarding your<br />
surgery. Find out if a second opinion is required by your insurance company. Verify information regarding<br />
facility charges and coverage for anesthesia and pathology services. Understand your responsibilities for<br />
out-patient surgery co-pays, deductibles, & out of pocket expenses. Please remember to get the name of<br />
the person you are speaking with in case you need to reference the conversation. Our office will obtain<br />
any necessary pre-authorization.<br />
While the care after surgery, or post-operative care, for most surgeries is part of the “global surgical fee”<br />
for 90 days, the surgical fees for some procedures, such as sinus surgery, only cover the fee for day of<br />
surgery. This means that a fee will be charged for post-operative visits for these types of surgery. If you<br />
belong to an insurance company that requires referrals from your Primary Care Physician you must<br />
obtain referrals for post-operative visits or you will be responsible for these charges. Please ask when<br />
scheduling your surgery if you will need referrals for your post-operative visits. If so, please obtain them<br />
prior to surgery.<br />
After myringotomy and ear tube surgery, post-operative visits are covered for 10 days. A hearing test<br />
(audiogram) is usually done post-operatively to assess hearing. It is not covered under the surgical fee.<br />
If your insurance plan requires a referral then you need to obtain one for the hearing test.<br />
CT Scans for Surgery<br />
If you have undergone CT scanning prior to your surgery, they are needed for your surgery. For example,<br />
sinus CT scans for sinus surgery, neck CT scans for neck surgery, and temporal bone CT scans for ear<br />
surgery. If these are in your possession please bring them to us prior to the date of surgery. Without<br />
these scans your surgery will be cancelled. Please check with us when scheduling your surgery to verify<br />
that we have your CT scans.<br />
Disability Forms<br />
We will be happy to complete disability form(s) for you. All disability forms are filled out and mailed<br />
within 5 business days. There is a $10.00 fee per form.<br />
Cosmetic Surgery Fees<br />
If your surgery requires a Cosmetic Fee, please make your payment 10 days prior to the surgical date. If<br />
less than 10 days please pay by cash, credit card, or a certified check.<br />
Medication<br />
Do NOT take aspirin or aspirin products (NSAIDS, including Advil & Motrin) for two weeks prior to and<br />
after surgery. Tylenol is safe to take. Inform your doctor of any medications that you take on a regular<br />
basis, especially blood thinners (Warfarin, Coumadin, Plavix, Pradaxa).<br />

<strong>Pre</strong>-Admission Testing<br />
<strong>Pre</strong>-Admission testing is not required for every patient having surgery. It may include bloodwork, EKG,<br />
Chest X-ray and/or Medical Clearance. Our office will notify you when scheduling your surgery if any<br />
pre-admission testing will be required.<br />

If your Insurance Company requires you to have your bloodwork done at a LabCorp facility, you must go<br />
to LabCorp and cannot have your blood drawn at either of the above locations.<br />
Time of Surgery<br />
The Ambulatory Surgical Center will contact you 1-2 days prior to surgery to advise you what time to<br />
arrive for the procedure. Surgical cases are done by order of age from youngest to oldest. Please make<br />
sure that we have your current phone number(s).<br />
Arrange for a responsible adult to drive you home and remain with you for the first 24 hours after<br />
surgery.<br />
Nothing to eat or drink after midnight the night before your surgery. This includes water, hard candy, and<br />
gum. Please call the office for specific instructions for children.<br />
Notify your physician prior to surgery with any questions or if you experience a change in your physical<br />
condition such as a cold, cough, or fever.<br />
Day of Surgery<br />
• Arrive on-time to Ambulatory Surgery Center.<br />
• Wear comfortable clothing.<br />
• Leave jewelry and valuables at home.<br />
• Bring your ID and insurance information.<br />
