How to Complete the Australian Standard Transfer Form
1. Description of Securities
e.g. Fully Paid Ordinary Shares or Franked Unsecured Equity Listed Securities (FUELS). A separate transfer form is required for
each class of share.
2. Quantity
Number of shares being transferred (in both words and figures).
3. Full Name(s) of Transferor(s)
Please note that Deceased Estates should be entered as follows:
“(the full names of the executors) as Executors of the Estate of (full name of deceased shareholder)”.
4. Shareholder Reference Number (SRN) of Transferor(s)
The SRN can be found on the Issuer Sponsored Holding Statement.
5. Consideration & Date of Purchase
This is the full amount paid in settlement of the transfer of shares. Purchase should reflect the market price of shares as at the date of
purchase. Market values are quoted in the financial or business section of daily newspapers.
6. Full Name(s) of the Transferee(s)
Full names of all transferees (a maximum of three joint shareholders). Shares may not be registered in the name(s) of a partnership or
business name, a fund or a trust, an estate or deceased person or a person under 18 years of age.
