Adding new programs.<br />

Making things better.<br />

Over the past several years,<br />

we have exp<strong>and</strong>ed the<br />

choice <strong>of</strong> sports, fi tness,<br />

<strong>and</strong> arts programs <strong>of</strong>fered<br />

in these pages. The most<br />

obvious way was the<br />

expansion <strong>of</strong> our aquatics<br />

programs, thanks to the<br />

construction <strong>of</strong> two indoor<br />

pools at the Aquatic <strong>and</strong> Community Centre in 2011. We have also<br />

introduced other programs ourselves or with partners like the launch <strong>of</strong><br />

the Côte Saint-Luc Dramatic Society or the JHoops basketball program.<br />

The Parks <strong>and</strong> Recreation Department collaborates with its<br />

community partners in providing cool facilities <strong>and</strong> programs,<br />

accessible to all residents <strong>and</strong> contributing to a vibrant, healthy<br />

<strong>and</strong> active community.<br />

Aside from our programs, we have also enhanced many <strong>of</strong> our<br />

parks. Sometimes the change is relatively minor, like the addition<br />

<strong>of</strong> a pétanque court at Rembr<strong>and</strong>t Park. Other times, we completely<br />

renovated the space, like Nathan Shuster Park, which we re-opened<br />

last fall to outst<strong>and</strong>ing reviews.<br />

We're always fi nding ways to improve things. We never sit still <strong>and</strong>—with<br />

so many ways to get active—neither should you. Browse these pages<br />

to fi nd an activity for you, your kids, or even your parents. There is lots<br />

to do this spring <strong>and</strong> summer in Côte Saint-Luc. Get active <strong>and</strong> stay<br />

V-E Day Commemoration Ceremony<br />

Veterans Park (next to the library)<br />

The annual Victory in Europe (V-E) Day commemoration ceremony<br />

is organized by the City <strong>of</strong> Côte Saint-Luc <strong>and</strong> the Brigadier<br />

Frederick Kisch Branch 97 <strong>of</strong> the Royal Canadian Legion. The<br />

event commemorates the victory <strong>of</strong> the Allies over Nazi Germany.<br />

Blood Donor Clinic<br />

How to Register<br />

You can register for Côte Saint-Luc Parks <strong>and</strong> Recreation<br />

programs in person, by mail or fax, or on our secure website.<br />

In person: See Registration Dates for location information.<br />

Registration fees must be paid in full at the time <strong>of</strong> registration,<br />

unless otherwise stated in this guide. We accept cash, Visa,<br />

MasterCard, Interac, or cheque made payable to the City <strong>of</strong><br />

Côte Saint-Luc. You may also deposit your registration form <strong>and</strong><br />

payment after hours in the mail slot next to the main doors.<br />

Register by mail: City <strong>of</strong> Côte Saint-Luc, 7500 Mackle Rd., Côte<br />

Saint-Luc, Quebec, H4W 1A6.<br />

Register by fax: Complete the registration form indicating your<br />

credit card number <strong>and</strong> expiration date. Payment by credit card<br />

only. Fax to 514-485-8629.<br />

Register online: If you or a family member (spouse, sibling, child)<br />

have previously registered for a program, call 514-485-6806<br />

weekdays from 8:30 am to 4:30 pm to receive your username<br />

<strong>and</strong> password.<br />

If you (or your family) are registering for a program for the first time,<br />

visit CoteSaintLuc.org/RegisterOnline to create an account for<br />

you <strong>and</strong> all the members <strong>of</strong> your household. You will need to provide<br />

pro<strong>of</strong> <strong>of</strong> residency by fax (514-485-8629) or e-mail (recreation@<br />

cotesaintluc.org). We will then e-mail you within two business days<br />

to confirm that we received your pro<strong>of</strong> <strong>of</strong> residency <strong>and</strong> will activate<br />

your account. Then visit CoteSaintLuc.org/RegisterOnline to sign<br />

in to your account. You can select your program <strong>and</strong> pay for it using<br />

our secure financial transaction website.<br />

Registration Procedures & Policies<br />

Responsibility<br />

Participants are fully responsible for any damage caused to City<br />

<strong>of</strong> Côte Saint-Luc facilities <strong>and</strong> equipment.<br />

Cancellations<br />

All programs are subject to change. If a program is cancelled due<br />

to minimum registration not being attained, existing registrants<br />

will receive a full refund.<br />

General Procedures <strong>and</strong> Policies<br />

Fees, locations <strong>and</strong> dates are subject to change.<br />

1. Most programs have limited registration <strong>and</strong> all registrations<br />

are on a fi rst-come, fi rst-served basis.<br />

2. Registration fees cannot be transferred from one program to<br />

another or one person to another.<br />

3. Taxes do not apply to registrants age 13 <strong>and</strong> younger.<br />

4. Please note that a $35 service charge will be charged on all<br />

cheques returned from the bank due to insuffi cient funds.<br />

5. Cheques to pay for programs must be made payable to the<br />

City <strong>of</strong> Côte Saint-Luc.<br />

6. All programs <strong>of</strong>fered by the Parks <strong>and</strong> Recreation Department<br />

<strong>of</strong>fer a 20 percent discount (rounded to the nearest dollar)<br />

to seniors age 60 or older. <strong>Program</strong>s <strong>of</strong>fered by our partner<br />

organizations are excluded from this policy. Most programs<br />

have limited registration <strong>and</strong> all registrations are on a fi rstcome,<br />

fi rst-served basis. *Unless otherwise stated by NSR (No<br />

Senior Rate).<br />

<strong>Program</strong> Pro-Rating Policy<br />

Those who register for a program will pay full price until the<br />

program’s third week. Thereafter, the price <strong>of</strong> the program<br />

is pro-rated based on the number <strong>of</strong> classes remaining <strong>and</strong> a<br />

10 percent late registration fee will apply. The 10 percent fee<br />

is based on the total original cost <strong>of</strong> the program. Some sports<br />

programs are not pro-rated.<br />

General Refund Policy<br />

Any refunds granted are entirely at the discretion <strong>of</strong> the City<br />

<strong>of</strong> Côte Saint-Luc.<br />

Requests for refunds will only be considered upon the City <strong>of</strong><br />

Côte Saint-Luc receiving a written request prior to the date at<br />

which one-third <strong>of</strong> the program has been completed, subject to<br />

the medical reason exception stated below. All refunds granted<br />

will be subject to an administrative charge <strong>of</strong> 20 percent <strong>of</strong>f the<br />

total cost <strong>of</strong> the program, <strong>and</strong> any refunds granted will be prorated<br />

based solely on the date the City <strong>of</strong> Côte Saint-Luc receives<br />

the written refund request.<br />

The above policy is applicable for all recreation programs (except<br />

day camp <strong>and</strong> playgroup programs) administered by the City <strong>of</strong><br />

Côte Saint-Luc.<br />

Refunds Due to Medical Reasons<br />

Any refunds requested for medical reasons will be considered<br />

at any time, provided a valid note from a physician accompanies<br />

the written request.<br />

Policies<br />

If granted, the refund will be pro-rated on the<br />

unused portion <strong>of</strong> the fee <strong>and</strong> an administrative charge equal to<br />

10 percent <strong>of</strong> the total cost <strong>of</strong> the program will be applied in all<br />

cases.<br />

Aquatics Refund Policy<br />

Requests for refunds will only be considered upon the City <strong>of</strong><br />

Côte Saint-Luc receiving a written request to withdraw from a<br />

program. All refunds granted will be subject to an administrative<br />

charge <strong>of</strong> 20 percent <strong>of</strong> the published program fee.<br />

<strong>Program</strong> Fees: For any withdrawals from competitive teams, a<br />

refund <strong>of</strong> the <strong>Program</strong> Fees will be issued pro–rated to the end<br />

<strong>of</strong> the month where the request for withdrawal was received. No<br />

refunds will be issued after April 30, 2015.<br />

Affiliation Fees: non-refundable.<br />

Club Fees: The balance <strong>of</strong> Club Fees in the competitor’s<br />

account at the time <strong>of</strong> withdrawal is fully refundable, or can be<br />

carried over to be used towards the following season.<br />

Volunteer Fees: non-refundable.<br />

Note: Only programs managed by the Parks <strong>and</strong> Recreation Department are<br />

listed online. The registration procedure for activities organized by partner<br />

organizations is set by those organizations.<br />

Swimming Lessons<br />

Red Cross Pre-School<br />

Swimming Lessons<br />

Certifi ed by the Red Cross, the Pre-School Swimming Lessons<br />

are for children aged 4 months to 5 years. Children enter at one<br />

<strong>of</strong> eight levels based on age <strong>and</strong> the progress based on either<br />

age or ability. Eight levels increase opportunity for parents <strong>and</strong><br />

their children to take swimming <strong>and</strong> water safety lessons at an<br />

earlier age.<br />

Red Cross Swim School<br />

Swimming Lessons<br />

Certifi ed by the Red Cross, the Swim School program is for ages 6<br />

to 16. Swimming <strong>and</strong> water safety skills are taught in the water so<br />

that kids are always active. This approach promotes success <strong>and</strong><br />

encourages lifelong fi tness. A focus on individual achievement<br />

creates a cycle <strong>of</strong> success geared to all kids.<br />

I Can Swim Pre-school lessons will introduce children to the<br />

water. The goal is to ensure children are safe, relaxed <strong>and</strong><br />

having fun while still learning <strong>and</strong> improving in the water. For<br />

children 4 months to 5 years old.<br />

I Can Swim Swimming Lessons<br />

A progressive three-stage program. Teaches swimmers 54<br />

important water skills, <strong>and</strong> focuses on seven major skill<br />

progressions: butterfl y, breaststroke, backstroke, free-style, water<br />

safety, entries <strong>and</strong> turns. Swimmers learn to put together whole<br />

skill-sets, one step at a time. For children 5 to 16 years old.<br />

I Can Swim Fast<br />

Stroke drills, endurance building <strong>and</strong> goal-setting give swimmers<br />

the knowledge <strong>and</strong> confi dence to swim effi ciently <strong>and</strong> FAST.<br />

Focus is on skill refi nement <strong>and</strong> stroke enhancement to prepare<br />

swimmers for swim team. Prerequisites: Children must have<br />

successfully completed I Can Swim Stage 2 <strong>and</strong>/or Red Cross<br />

level 7. For children 8 to 16 years old.<br />

Adult Swimming Lessons<br />

Swimming lessons will enable adults to learn necessary<br />

swimming techniques as well as refi ne existing skills through<br />

effi cient <strong>and</strong> personalized teaching.<br />

Private Lessons<br />

One on one private lessons for children or adults. Lessons are<br />

taught by our experienced coaching staff <strong>and</strong> are customized<br />

Lifeguard Training<br />

Lifeguard in Training Course<br />

Learn from <strong>and</strong> assist a certifi ed lifesaving instructor in all<br />

operations <strong>of</strong> the Parkhaven Outdoor Pool. The Bronze Medallion<br />

<strong>and</strong> Bronze Cross courses are included in this program.<br />

Prerequisites: Age 13 <strong>and</strong> must have completed the highest level<br />

<strong>of</strong> a certifi ed swimming course.<br />

Bronze Medallion Course<br />

The Bronze Medallion course teaches an underst<strong>and</strong>ing <strong>of</strong><br />

the lifesaving principles <strong>of</strong> water rescue education judgment,<br />

knowledge, skill <strong>and</strong> fi tness. Prerequisites: Age 13 <strong>and</strong> must<br />

have completed the highest level <strong>of</strong> a certifi ed swimming course.<br />

Bronze Cross Course<br />

The Bronze Cross course is designed for lifesavers who want the<br />

challenge <strong>of</strong> more advanced training including an introduction<br />

to safe supervision in aquatic facilities. They will learn the<br />

difference between preventing <strong>and</strong> responding, the reasons<br />

behind emergency procedures <strong>and</strong> team work. Prerequisites:<br />

Age 13 <strong>and</strong> must have completed Bronze Medallion.<br />

St<strong>and</strong>ard First Aid Course<br />

Comprehensive two-day course <strong>of</strong>fering fi rst aid <strong>and</strong> CPR skills<br />

for those that need training due to work requirements or who<br />

want more knowledge to respond to emergencies at home.<br />

Prerequisite: Age 14.<br />

National Lifeguard Course<br />

This course will give you a legal certifi cate for lifeguarding<br />

throughout the country <strong>and</strong> the only lifeguard certifi cate<br />

recognized by the province <strong>of</strong> Quebec. Prerequisites: Age 16 <strong>and</strong><br />

must have completed St<strong>and</strong>ard First Aid <strong>and</strong> Bronze Cross.<br />

Competitive Teams<br />

Registration for any <strong>of</strong> the four competitive team programs<br />

included access to the pool at any open swim times <strong>and</strong> the<br />

fi tness room during hours <strong>of</strong> operation: water polo, synchronized<br />

swimming, masters <strong>and</strong> youth teams. Season fees also include<br />

Parkhaven Outdoor Pool membership for the 2015 season into<br />

competitive teams <strong>and</strong> pool access.<br />

Youth Triathlon Team<br />

Swim, bike <strong>and</strong> run. This new Youth Triathlon program is a unique<br />

opportunity to work with experienced coaches in partnership with<br />

Tri-O-lacs Triathlon Club. Participants will develop <strong>and</strong> improve<br />

skills <strong>and</strong> techniques in all three disciplines.<br />

Water Polo Lessons/Team<br />

Swimmers will learn the basics <strong>of</strong> the game; rules, ball control<br />

skills <strong>and</strong> basic strategy all while improving swimming endurance.<br />

Teams will compete within the North Shore Aquatic Association<br />

(NAA) if numbers are suffi cient. Aimed at swimmers ages<br />

10 to 16. Swimmers must be comfortable in deep water. They must<br />

be able to complete a 50 metre non-stop swim <strong>and</strong> 5 minutes <strong>of</strong><br />

treading water.<br />

Synchronized Swimming Lessons/Team<br />

Young swimmers will learn the fundamentals <strong>of</strong> the sport that<br />

combines physical endurance with grace <strong>and</strong> beauty. Over the<br />

summer, swimmers will develop <strong>and</strong> create a routine <strong>and</strong> may<br />

compete in a North Shore Aquatic Association (NAA) competition.<br />

For individuals aged 5 to 15. Swimmers must be comfortable in<br />

deep water.<br />

Masters Team<br />

The Masters team is for swimmers aged 18 or older who want<br />

to improve their swimming strokes, technique or general fi tness.<br />

Swimmers <strong>of</strong> all levels are welcome; no prior competitive experience<br />

required.<br />

Masters Triathlon Team<br />

Swim, bike <strong>and</strong> run. This Masters Triathlon is for adults aged 18<br />

or older who want to challenge themselves in three disciplines or<br />

newcomers to triathlon wishing to get fi t, stay healthy <strong>and</strong> improve<br />

skills <strong>and</strong> competencies. Participants <strong>of</strong> all skill levels are welcome,<br />

no competitive experience required.<br />

Youth Swim Team<br />

Instruction on developing skills <strong>and</strong> endurance <strong>of</strong><br />

technical elements with the aim <strong>of</strong> competing at<br />

Regional, Provincial <strong>and</strong> National levels. There is an<br />

opportunity for physically <strong>and</strong> intellectually challenged<br />

swimmers to join the team <strong>and</strong> compete in Para <strong>and</strong><br />

Special Olympics competitions. Swimmers must be<br />

comfortable in deep water; must be able to complete<br />

50 metres <strong>of</strong> continuous swim.<br />

Summer Swim Team<br />

The Côte Saint-Luc Sharks summer swim team competes<br />

as part <strong>of</strong> the North Shore Aquatic Association (NAA).<br />

They will be competing in Division B in June 2015. The<br />

team welcomes swimmers <strong>of</strong> all ages; swimmers must be<br />

comfortable in deep water <strong>and</strong> be able to complete a non-stop<br />

25 metre swim. For individuals aged 5 to 15.<br />

Aquafitness <strong>Program</strong>s<br />

Similar to aqua-cycle but designed for those that need a low<br />

Aqua Arthritic<br />

Aquatic physical conditioning for people with joint problems.<br />

The emphasis is on stretching <strong>and</strong> muscle toning to improve<br />

<strong>and</strong> retain a good range <strong>of</strong> motion.<br />

Pre-Natal<br />

Safe <strong>and</strong> effective shallow water aquafi tness class designed<br />

for women to obtain <strong>and</strong> maintain a healthy, active lifestyle<br />

during pregnancy.<br />

Baby Swim<br />

Getting baby used to the water early is important for safety<br />

<strong>and</strong> it can be great fun too! Babies don’t have the proper<br />

coordination to swim until age two or three. Build baby’s<br />

confi dence in the water. It’s fun exercise for both <strong>of</strong> you.<br />

Inter-community “A” <strong>and</strong> “B” Baseball<br />

Now in its 30th year, the Inter-community Baseball league features<br />

home <strong>and</strong> away games against other communities including<br />

Hampstead, Montreal West, TMR <strong>and</strong> Westmount. Players receive<br />

full uniforms <strong>and</strong> qualifi ed coaches teach players all aspects <strong>of</strong><br />

the game. Children who wish to play either “A” or “B” baseball<br />

must attend the “A” try-outs. Children who do not qualify for an “A”<br />

team will be able to play on a “B” team. Tryouts begin in early April.<br />

Côte Saint-Luc Figure Skating Club<br />

The Côte Saint-Luc Figure Skating Club is a member <strong>of</strong> Skate Canada. Six-week group lessons for children ages 5 to 12 are<br />

Basketball<br />

These basketball programs are organized by JHOOPS. The Spring<br />

Basketball <strong>Program</strong> allows kids from grades 2 to 4 to learn<br />

basketball skills such as dribbling, passing, <strong>and</strong> shooting, <strong>and</strong><br />

the fundamental rules <strong>of</strong> the game. The Spring Basketball League<br />

allows kids in grades 5 to 7 to play on a league <strong>and</strong> experience<br />

Proset Autism Special Needs<br />

Tennis <strong>Program</strong><br />

Proset Autism is a new special needs sports program being<br />

<strong>of</strong>fered at the Côte Saint-Luc Gymnasium on Saturdays from 1 pm<br />

to 2 pm by the Parks <strong>and</strong> Recreation Department. This program<br />

is specifi cally for children <strong>of</strong> special needs who wish to learn <strong>and</strong><br />

play tennis in a structured <strong>and</strong> supervised setting.<br />

With Proset Autism, kids will learn to respect one another’s<br />

boundaries <strong>and</strong> limitations through structured games. By<br />

developing small incremental stages, participants will learn the<br />

unfolding <strong>of</strong> a game <strong>and</strong> gradually, with ample opportunity to<br />

learn to make their own decisions <strong>and</strong> their own physical actions<br />

independently.<br />

Children’s Fitness <strong>and</strong> Art Tax Credits<br />

The Government <strong>of</strong> Canada promotes physical fitness <strong>and</strong> artistic activities <strong>of</strong> children under the age <strong>of</strong> 16. This policy currently<br />

allows parents to claim tax credits. <strong>Program</strong>s <strong>of</strong> a duration <strong>of</strong> eight weeks or more, <strong>of</strong>fered at a minimum <strong>of</strong> one session per<br />

week, are currently eligible for these credits. Parents can claim up to $1,000 for sports programs, such as baseball, all, basketball,<br />

swimming, <strong>and</strong> more. For arts programs such as music, pottery, <strong>and</strong> more, parents can claim up to $500. Parents <strong>of</strong> children with<br />

Youth Action <strong>Program</strong><br />

The Youth Action <strong>Program</strong> is a club for tweens <strong>and</strong> teens aged<br />

11 to 17. Headquartered at the ACC, the program <strong>of</strong>fers many<br />

activities for teens, such as rock climbing, bowling, casino night,<br />

movie <strong>and</strong> pizza night, trips to La Ronde <strong>and</strong> Laser Quest <strong>and</strong><br />

much more during the year.<br />

Every Tuesday <strong>and</strong> Thursday from 4 pm to 9 pm, the Youth Action<br />

<strong>Program</strong> has exclusive use <strong>of</strong> the ACC Game Room <strong>and</strong> all its<br />

equipment, including:<br />

*a pool table<br />

*a foosball table<br />

*a ping pong table<br />

*an air hockey table<br />

*a large screen TV<br />

*3 gaming consoles (Xbox, PS3 <strong>and</strong> Wii)<br />

Côte Saint-Luc operates three day camps in our city parks <strong>and</strong><br />

other outdoor <strong>and</strong> indoor facilities, such as the Parkhaven<br />

Outdoor Pool, Aquatic <strong>and</strong> Community Centre indoor pools,<br />

Côte Saint-Luc Gymnasium <strong>and</strong> more.<br />

For city-run day camps, there are four two-week sessions:<br />

A: June 29 to July 10, 2015<br />

B: July 13 to July 24, 2015<br />

C: July 27 to August 7, 2015<br />

D: August 10 to August 21, 2015<br />

General Information About Day Camps<br />

All cheques must be made payable to the City <strong>of</strong> Côte Saint-Luc.<br />

Payment by Visa, MasterCard <strong>and</strong> Interac are also available for<br />

the deposit portion only. The post-dated portion can only be paid<br />

by cheque.<br />

Upon registration, a deposit <strong>of</strong> 50% <strong>of</strong> the total amount owing is<br />

required in addition to a post-dated cheque, dated for May 15,<br />

2015, covering the balance.<br />

The deadline to register for any session is the previous Wednesday<br />

at noon, in order to allow for time to fi nalize group lists. Past this<br />

deadline, registration will be accepted pending availability. A 5%<br />

premium will apply.<br />

An early bird registration fee will be in effect from April 1 to 22,<br />

2015. The regular registration fee will be in effect as <strong>of</strong> April 23.<br />

The regular fee is 10% higher than the early bird fees.<br />

Early drop-<strong>of</strong>f / Late pick-up service<br />

An 8 am early morning drop-<strong>of</strong>f service is provided for parents at<br />

an extra cost <strong>of</strong> $16 (resident rate) or $20 (non-resident rate) per<br />

week. A late pick-up service is provided for parents at an extra<br />

cost <strong>of</strong> $20 (resident rate) or $25 (non-resident rate) per week<br />

until 5:30 pm. All children must be dropped <strong>of</strong>f <strong>and</strong> picked up<br />

from the park at which they are registered.<br />

Additional supervision for Day Camps 2015<br />

Children who require additional supervision to participate in the<br />

day camp programs should contact the Parks <strong>and</strong> Recreation<br />

Department. Funding may be available through the community<br />

organization Altergo. The deadline to submit an application for<br />

funding is March 13, 2015.<br />

The Tennis <strong>and</strong> Sports Academy (TSPA) operates this day<br />

camp. Campers have up to three hours <strong>of</strong> instructional<br />

tennis <strong>and</strong> there is a maximum <strong>of</strong> eight campers per one<br />

tennis instructor. Campers also enjoy a fi eld game (<strong>of</strong>ten<br />

Learn how to perform cardiopulmonary resuscitation<br />

(CPR) in case <strong>of</strong> cardiac arrest for both adult, child,<br />

WAIVER - CONDITION OF PARTICIPATION IN ANY PARKS AND RECREATION DEPARTMENT PROGRAM<br />

Risks/Release: As a condition <strong>of</strong> attendance at/use <strong>of</strong> a City <strong>of</strong> Côte Saint-Luc (“City”) facility or participation in a City activity/program, I hereby assume all direct <strong>and</strong> indirect, foreseeable <strong>and</strong> unforeseeable risks relating thereto for me <strong>and</strong> my minor child. I hereby<br />

release the City its elected <strong>of</strong>ficials, employees, agents, contractors, <strong>and</strong> volunteers, <strong>and</strong> their successors (collectively, “City Releasees”) from, <strong>and</strong> waive <strong>and</strong> renounce to, every claim <strong>and</strong> liability <strong>of</strong> whatsoever nature, whenever so arising, for loss, damage or injury to my<br />

or my minor child’s person <strong>and</strong> property (including theft). I agree to defend, indemnify <strong>and</strong> hold the City Releasees harmless from every such claim in principal, interest <strong>and</strong> costs. Emergency: If a City representative judges there is a medical emergency affecting me or<br />

my minor child I authorize them to call 9-1-1 <strong>and</strong>/or EMS, for police/ambulance/emergency medical services <strong>and</strong> allow the City to provide emergency medical intervention (including medication/treatment) at my sole cost <strong>and</strong> peril, <strong>and</strong> the below Release shall apply.<br />

Medical Conditions: I will complete a separate form provided by the City (where required) <strong>and</strong> declare all medical conditions, medicaire number, mobile phone number, <strong>and</strong> an emergency contact person with number, without the City incurring any liability for knowledge<br />

<strong>of</strong> a pre-existing medical condition. I or my minor child shall not attend a City or program or activity when symptoms <strong>of</strong> illness are present. If I am asked to leave/withdraw, or to retrieve/withdraw my minor child, for reasons <strong>of</strong> conduct or illness, I will do so promptly<br />

without dispute or right to refund or reimbursement. Use <strong>of</strong> Image etc: The City may photograph or videograph me or my minor child <strong>and</strong> may use my or my minor child’s image <strong>and</strong>/or voice clips, photograph or videograph in its discretion in/on any media in perpetuity<br />

without any payment or further consent. I hereby waive my/our moral rights, <strong>and</strong> assign full copyright, in favour <strong>of</strong> the City in connection herewith. False Information: Any false information provided at any time to the City will automatically annul this registration, without<br />

recourse or penalty against, or refund or reimbursement by the City. Full Payment <strong>and</strong> Refund Policy: I acknowledge that full payment is required at the time <strong>of</strong> registration <strong>and</strong> prior to commencement <strong>of</strong>, or participation in, any City program or activity. I acknowledge<br />

that I am not entitled to refund or reimbursement <strong>of</strong> any amounts paid except as permitted under applicable City policies as published by the City at the date <strong>of</strong> my payment.<br />

REFUND POLICY<br />

Any refunds granted are at the entire discretion <strong>of</strong> the City <strong>of</strong> Côte Saint-Luc. Requests for refunds will only be considered upon the City <strong>of</strong> Côte Saint-Luc receiving a written request prior to the date at which one third <strong>of</strong> the program has been completed, subject to the<br />

medical reason exception. All refunds granted will be subject to an administrative charge <strong>of</strong> 20% <strong>of</strong>f the total cost <strong>of</strong> the program <strong>and</strong> any refunds granted will be pro-rated based solely on the date the City <strong>of</strong> Côte Saint-Luc receives the written refund request. The above<br />

policy is applicable for all recreation programs (except day camp <strong>and</strong> playgroup programs) administered by the City <strong>of</strong> Côte Saint-Luc.
